By your signature below, New System Owner represents and warrants that ownership of the Enphase PV
System(s) identified above (the “System”) has been transferred from the previous system owner to the
New System Owner on the Date of PV System Ownership Transfer listed above and that the System
remains at the original end user location.
New System Owner will, at its own expense, defend and indemnify Enphase and its affiliates from and
against any claims, suits, losses, damages, liabilities, costs, and expenses (including reasonable
attorneys’ fees) brought by any other third party resulting from or relating to the representations,
warranties, and transfer of ownership hereunder.
If New System Owner requests the continuation of any remaining Warranty with respect to the Enphase
Products in the System (“Continuation of Coverage”), such Continuation of Coverage will become
effective upon receipt by Enphase of this Form and a Continuation of Coverage Fee of €100 (payable by
check) no later than 30 days from the Date of PV System Ownership Transfer. Such fee shall be sent to
Enphase Customer Service at the address below. You will be notified via e-mail when your Continuation
of Coverage is effective. Enphase reserves the right to request evidence of the Date of PV System
Ownership Transfer.
