Team Carnival Game/Activity Registration The Relay For Life of Western Berks is proud to host "Hope Carnival" as this year's on-site fundraiser. As always, teams are not required to participate in the on-site fundraiser, but we sure hope you will! In order to minimize the leg-work our teams need to do, the Committee has already done the research and has outlined it in this document. Teams wishing to participate in Hope Carnival should complete the last page of this form and return it at the March team captain meeting. By pooling our resources and purchasing in bulk, we will be able to offer the best possible prizes at the lowest possible cost. To further attempt to reduce costs - will be TRY to obtain a sponsor to underwrite the cost of the prizes or obtain donations. We will also TRY to find a vendor who will accept "returns" of unused prizes to further reduce our overall expense. Teams who wish to participate in the prize tent are requested to do the following: Issue tickets (in lieu of prizes) to the winners of their games. Guidelines will be provided to ensure uniformity so that, i.e. everyone's "small" prize will be comparable. Work in the prize tent for a minimum of 2 hours (schedule will be created and you will have the opportunity at the May team captain meeting to sign up for the hours you will work). Team spirit points will be awarded for extra hours worked :) Contribute between 10-25% of your on-site game/activity sales to fund the prize tent. Final percentage will be determined by the number of teams participating in the prize tent. 2 Team Name: Team Captain: Check all that apply: My team was a 2009 RFL of Western Berks Pace-Setter Team.
