Birthday Party Agreement
1. A $75 non-refundable deposit is required to confirm your reservation.
2. Helmets are encouraged but not provided. Please bring your own helmet.
3. There is no admittance to the Party Room before your reserved time.
4. Confetti, popcorn, silly string, or pinatas are NOT allowed inside the premises.
5. Warm clothing, mittens and calf-length socks are recommended.
6. No outside food or alcohol is allowed for parties or private events. Other food options are
available to order upon request. A $100 surcharge will be applied for any alternative food
or beverages not provided by Zizzo’s.
7. A $100 cleaning fee will be added if any terms of this agreement are violated.
Release and Waiver Agreement
I, _______________________________________________ agree that Ice in Paradise, its
employees, volunteer staff, and executive committee is not in any way responsible for any loss
or damage sustained by me or persons in my group, including injury to persons or property. It is
further understood that participating in ice skating and ice sports carries inherent risks that
cannot be eliminated regardless of care taken. Furthermore, I agree to abide by the rules and
policies set forth by Ice in Paradise.

Photo Release
Permission to Use Photographs: I grant to Ice in Paradise, its representatives and employees the
right to take photographs of me and my party in connection with the above subject. I authorize
Ice in Paradise, its assigns and tranferees to copyright, use and publish the same in print and/or
electronically. I agree that Ice in Paradise may use such photographs of me with or without my
name and for any lawful purpose, including for example such purposes as publicity, illustration,
advertising, and Web content.
