GRANHOLM MARIANNE UDOW GOVERNOR DIRECTOR September 4, 2007 Stacy Raterink 5730 Roanoke Portage, MI 49024 RE: Registration # DF390037146 Stacy Raterink 5730 Roanoke Portage, MI 49024 Dear Mrs. Raterink: This letter is a follow-up to the Agency’s findings regarding the 10% sample inspection conducted at your home on 08/24/2007. The purpose of this inspection was to determine compliance with applicable licensing statutes and administrative rules for Family and Group Day Care Homes. A. Description of Facility Physical space has been approved in this private residence as follows: Main floor – Living Room 14’ x 18’(252 Sq. ft.); Dining/Kitchen room 20’ x 12’ (240 Sq. Ft.); Master Bedroom 13’ x 15’ (195 Sq. Ft.); Computer Room 8’ x8’ (64SQ. Ft.) equals a total of 751 Square feet available for child use. Physical space will accommodate the requested capacity of 6 unrelated children. There is one bathroom available for use. The house is a single story dwelling built around 1953. Licensee stated the wood siding has been changed since the house was built and there is no lead paint. The residence is using a standard forced air type furnace for heating the residence. There is no wood burning type appliances in the residence. The residence has two standard door exits (front and back). Egress style windows are not required. The outdoor play area is the fenced backyard of the residence. A water hazard has been identified on the premises. An above ground style 3 feet high swimming pool is located in the backyard. The swimming pool is a seasonal type pool that is put up for the summer months only. A substantial fence has not been placed around the swimming pool. Other days and times are available by special arrangement. Childcare services primarily provided for infant – 5 years but will accept school age children. Registrant does provides breakfast, lunch, dinner, and snacks. Registrant does not participate in a USDA Food Program. Activities and play equipment provided for all ages enrolled. Activities include: free play; structured group story times and circle time activities; outside play; quiet time with quiet activities; limited TV viewing and art/craft time and special projects. The residence has the following pets: 6 dogs; 6 cats;9 birds; 1 guinea pig; and fish in two aquarium type tanks. The registrant has completed the following training: CPR and First Aid - 8/31/2006; Bloodborne Pathogen Awareness (1 hour). There are no variances requested or approved at this time. B. Rule/Statutory Violations The violations that were found are: R 400.1903 Caregiver Responsibilities. (1) A caregiver shall be responsible for all of the following provisions: (f) Have a written and signed agreement with a responsible person who is 18 years of age or older to provide care and supervision for children during an emergency situation. A written agreement with the designated emergency person is not on file. R 400.1905 Training. (6) CPR and first aid training shall be maintained in the following manner: (a) Each year for CPR. CPR Certification has expired. R 400.1920 Outdoor play area and equipment. (5) When swings, climbers, slides, and other similar play equipment with a designated play surface above 30 inches are used, they shall: (a) Not be placed over concrete, asphalt, or a similar surface, such as hard-packed dirt or grass. 2 (b) Be safe, in good repair, and age-appropriate. The play structures must have loose fill placed under and around them. There is an old style teeter-totter in the back yard. It is highly recommended that you remove this piece of equipment. It presents a pinching hazard and a fall hazard. R 400.1921 Water hazards and water activities. (1) The caregiver shall ensure that barriers exist to prevent children from gaining access to any swimming pool, drainage ditch, well, natural or constructed pond or other body of open water located on or adjacent to the property where the child care home is located. Such barriers shall be of a minimum of 4 feet in height and appropriately secured to prevent children from gaining access to such areas. The swimming pool requires a minimum 4 feet in height fence around it. The fence must be a substantial type so that children cannot knock it down. Another option is to remove the pool. You have a type of pool that is seasonal and can be emptied and removed from the area. R 400.1921 Water hazards and water activities. (10) A caregiver shall obtain, and keep on file, written permission from a child's parent for the child's participation in either of the following: (a) Before each outdoor water activity at a swimming pool, lake, or other body of water off the child care home premises. (b) Seasonally for water activities occurring on the child care premises. Permission from parents required for water activities involving any type of pool etc. R 400.1932 Home maintenance and safety. (2) All dangerous and hazardous materials or items shall be stored securely and out of the reach of children. The freezer is in an accessible area and must be kept locked. The cabinet over the toilet is accessible to children and contains hazards. There are medications stored on the nightstand in the master bedroom used by children for sleeping. 3 R 400.1936 Animals and pets. (5) Litter boxes, pet food and dishes, and pet toys shall be inaccessible to children. Food and water dishes must be inaccessible to young children. As technical assistance and consultation, it was observed that cats were walking on your kitchen counters and stove top where food is prepared. You stated that you sanitize this area before you do any food preparation. It is the department’s expectation that you maintain this practice for all food preparation involving any meal or snack time. R 400.1945 Fire; tornado; serious accident and injury plans. (1) A written plan for the care of children shall be established and posted for each of the following emergencies: (a) Fire evacuation. (b) Tornado watches and warnings. (c) Serious accident or injury. (d) Water emergencies, if applicable. Emergency plans were not in writing or posted. R 400.1945 Fire; tornado; serious accident and injury plans. (3) Fire drills shall be practiced at least once a month and a written record that includes the date and time it takes to evacuate shall be maintained. A fire drill log was not available at time of inspection. R 400.1945 Fire; tornado; serious accident and injury plans. (4) Tornado drills shall be practiced once a month, April to October, and a written record that includes the date shall be maintained. A tornado drill log was not available at time of inspection. Due to the violations identified in the report, a written corrective action plan is required. The corrective action plan is due 15 days from the date of this letter and must include the following: • • How compliance with each rule will be achieved. Who is directly responsible for implementing the corrective action for each violation. 4 • • • Specific time frames for each violation as to when the correction will be completed or implemented. How continuing compliance will be maintained once compliance is achieved. The signature of the responsible party and a date. A follow-up inspection may be made to verify compliance. Should the corrections not be made in the specified time, it may be necessary to reevaluate the status of your certificate of registration. The Agency provides technical assistance to meet the licensing requirements and consultation to improve services. Please feel free to contact me with any questions.
