Health Foundation project: HEF into
Lincus
£75,000 over 15 months to integrate
Health Equalities Framework into the
Lincus platform. Making it:

Easier to use
Easier to collate information
Indicate where remedial action is
required
Trialled at Hft services in
Gloucestershire (supported living) and
Bradford (day service).
Project began in November 2015.
What is Lincus?

Tablet-based, self-reporting
tool
•
Allows people to communicate how they are feeling
•
Hft and Rescon developed an LD version
•
Trialled in Devon, where health and wellbeing
scores increased

Outcomes from the Lincus pilot:

20% increase in emotional health and
wellbeing
•
Diagnosis of advanced osteoarthritis
•
Reduction in challenging behaviours

Lincus.

Following the first HEF assessment, actions were put into
place:
• Support plan changed to Health Plan and additions
made to topics including equipment
• Health being discussed at reviews indicated some
issues
• Signposted and referrals made to specialist OT
• Health screening referrals made to health
• Weight being monitored

style site to community-based
• Very anxious about the move

 What is the role of day services in social care?
 There isn’t a ‘one size fits all’ solution
 There’s more than one way to embed an
innovation
 Health and social care must integrate to fully
appreciate the benefits.
