Making it: • • • Easier to use Easier to collate information Indicate where remedial action is required Trialled at Hft services in Gloucestershire (supported living) and Bradford (day service). Project began in November 2015. What is Lincus? • Tablet-based, self-reporting tool • Allows people to communicate how they are feeling • Hft and Rescon developed an LD version • Trialled in Devon, where health and wellbeing scores increased Lincus data Outcomes from the Lincus pilot: • 20% increase in emotional health and wellbeing • Diagnosis of advanced osteoarthritis • Reduction in challenging behaviours Health Equalities Framework What is the HEF? Why do we do it? What difference does it make? Using HEF and Lincus to improve health and wellbeing HEF scores improved Bradford HEF scores First and second HEF scores for each individual are vertically in line, showing an overall reduction for all individuals. Shaun Shaun – Shaun attends the day service five days per week. He has complex needs and is a non-verbal communicator. He is a very sociable person and has an infectious laugh. Loud or sudden noises can be distressing for Shaun. Shaun Shaun has started with a cough and has been sneezing a lot today. Overall wellbeing can be tracked against certain events or occurrences such as worsening health. April 2016 – December 2016 Staff could also closely monitor Shaun’s overall wellbeing through the increased insight from Lincus. Shaun Following the first HEF assessment, actions were put into place: • Support plan changed to Health Plan and additions made to topics including equipment • Health being discussed at reviews indicated some issues • Signposted and referrals made to specialist OT • Health screening referrals made to health • Weight being monitored Caroline • Moving house from a large campus style site to community-based • Very anxious about the move Social score reflects unsuitable housing No Health Action Plan in place Caroline Successfully moved house • Huge support from family members Now accessing community amenities, such as libraries, cafes and social clubs. Making friends Conclusions so far What is the role of day services in social care? There isn’t a ‘one size fits all’ solution There’s more than one way to embed an innovation Health and social care must integrate to fully appreciate the benefits.
