APPLICATION PROCEDURE
Mount St. Mary's Seminary requires that all applicants to the Ordination Formation program complete and submit
all of the materials outlined below, including the Seminary's official application to be filled out below (even though
some of the information may duplicate materials already gathered by your diocesan vocations office).
Please review the list before mailing your application packet to us. Thank you.
These items must be received by the Admissions Office before a file can be circulated to the committee for consideration.
The Admissions Committee would like to receive completed files for review by early spring (when possible).
Please follow the detailed instructions provided with the materials below. "Bookmarks" are provided at the left for ease of navigation.
1. MSM Seminary Official Application (fill in the blanks electronically, then print form and sign after completion)

6. Letters of Recommendation from each of the following:
a) your pastor or a priest who knows you best (excluding your spiritual director/confessor)
b) the Rector of every seminary (or priestly formation program) previously attended, however briefly (if applicable)
c) the Superior of any religious community or order (giving release, if you have previously been in formation for an institute of
consecrated life or been a member of such an institute)
7. Results of the TOEFL exam, or its equivalent (if English is not your native language)
8. Results for any standardized tests, such as GRE, SAT, MAT, you may have taken (however taking such tests is not required)
9. Official church certificates - dated within the past six months and sent directly from the church - (photocopies are not acceptable)
a) Certificate of Baptism
b) Certificate of Profession of Faith (if received into full communion with the Catholic Church after prior valid Baptism in another church)
c) Certificate of Confirmation
10. Official copies of any marriage certificates, decrees of divorce (if applicable), decrees of nullity (if applicable), or spouse's
death certificate (if applicable) - if you were previously married, however briefly, in any way at all including common law
and civil ceremonies - (photocopies are not acceptable)
Please complete all forms as thoroughly and accurately as possible, reviewing them carefully before printing (single-sided).
Application materials must be sent to the Mount St. Mary's Seminary Admissions Office in a timely fashion;
however individual documents may be sent separately, as soon as they are available. The academic year begins mid-August.
The Seminary Admissions Committee often starts processing seminary application by January for fall admittance.
We cannot guarantee acceptance if application materials are received after July 15th.

PSYCHOLOGICAL EXAMINATION

The psychologist should send a confidential report of recent examination directly to Mount St. Mary's Seminary
(attn: Seminary Admissions, 16300 Old Emmitsburg Rd, Emmitsburg, MD 21727-7797)
This report must be received before the application can be considered by the admissions committee.
The following tests, or their equivalent, must be included in the psychological examination:
1. Clinical Interview - a face-to-face interview that includes a complete psychological history (family background, sexual
history, strengths and weaknesses)
2. Wechsler Abbreviated Scale of Intelligence (WASI) - assesses verbal and performance-based intellectual functioning
3. Minnesota Multiphasic Personality Inventory-2, revised personnel edition - complete review of Axis I disorders, along
with an alcohol/drug and addition potential scale (if necessary, further testing could be required)
4. Coolidge Assessment Battery - overview of Axis II disorders, plus executive functioning scales.
5. FIRO Awareness Scales/Myers-Briggs Type Indicator combination for purpose of determining fitness for ministry:
the first checks for interactional styles (e.g., reflective vs. active), the second for personality styles and temperaments.
6. Brown Attention-Deficit Disorder (ADD) Scales
7. Sexual Addiction Screening Test (S.A.S.T.) (if necessary, further testing could be required)
8. Internet Sex Screening Test
9. Rorschach Test combined with Personal Sentence Completion Inventory - these tests will overlap to monitor for
potential deviant sexual interests.
Evaluation of Candidates for the Seminary – Issues for Sexual Identity and Sexual Orientation
~ Checking a sexual history requires a significant amount of time if it is to be done thoroughly and well ~
1. Family of Origin
Goal: To obtain an understanding of broad family attitudes about sexuality. Among individuals, the single most important circumstance
determinative of affective sexual development is the family environment.
2. Prepubescent Sexual Development
Goal: To obtain an understanding of the candidate's earliest sexual feelings and experiences. Early sexual stimulation is a risk factor for later
sexual problems.
3. Sexual Abuse History
Goal: To determine if the candidate has experienced any sexual abuse and/or any exploitation. Sexual abuse which has been dealt with by
the individual and the family can give insights into the family and the candidate's role within it.
4. Puberty and Adolescence
Goal: To obtain an understanding of the candidate's sexual development during adolescence, particularly with regard to puberty experiences.
Sexual deviancy often shows its first signs in adolescence. Those with excessive sexual involvement in adolescence, as well as people
who report no sexual curiosity or sexual challenges during adolescence, may have sexual problems.
5. Sexual Orientation
Goal: To obtain an understanding of the candidate's awareness of his sexual orientation and his ability to live a balanced life with that
understanding.
6. Dating and Adult Sexual Activity
Goal: To obtain an understanding of the candidate's experience of dating and adult sexual activity. If a person's sexuality is not integrated
or not balanced in any way, this will frequently be indicated in his sexual behavior.
7. Pedophilia and other Problematic Sexual Behavior
Goal: To determine if the candidate has engaged in sexual deviance or problematic behavior. The presence of previous deviate sexual behavior
or a deviate sexual arousal pattern is cause for considerable concern and should be investigated as thoroughly as possible.
Mount St. Mary's Seminary urges that personal feedback should be given directly to the client by the psychologist after the report.

CRIMINAL BACKGROUND CHECKS
INFORMATION SHEET
~ please read carefully ~
All seminarians enrolled at Mount St. Mary's Seminary are required to be fingerprinted and
have a criminal background check, in compliance of the Archdiocese of Baltimore policies.
Fingerprinting records from outside the Archdiocese of Baltimore
do not satisfy this Archdiocesan requirement.
The procedure for fingerprinting is as follows:
1. As early as possible at the beginning of the semester, a specific date will be arranged by the seminary to conduct the
fingerprinting for any new seminarian(s). Compliance is mandatory.
2. The fingerprinting cards will be sent to the Criminal Justice Information System (CJIS) for a "Full Criminal Background
Check" including the national FBI check.
a. Although this may have already been done in the seminarian's own diocese/religious community, this is a separate
requirement pursuant to the Archdiocese of Baltimore regulations.
3. There is a one-time fee of approximately $45.00 per person. This fee for the fingerprinting costs will be billed to the
seminarian's diocese (or religious community).
4. After CJIS processes the background checks, they will send out two confidential official reports (one to the seminarian, and
the other to Mount St. Mary's Seminary).
a. NOTE: It is against Maryland Law for any seminary personnel to share any information contained in the official
"Full Criminal Background Check" report with any person or agency, either verbally or by duplicating records.
b. A representative of the (Arch) Diocese or Vocation Office may obtain a copy of this official report only by:
i. Obtaining an authorization number from CJIS. (for information on how to apply for the authorization
number see www.dpscs.state.md.us/ or call 410-764-3638 or 410-585-3638)
OR
ii. Requesting that the seminarian relinquish the official copy of the background check which he
received as his personal property.
This could save the sponsoring diocese or Vocation Office the task of having to make an application
for a CJIS authorization number, however this must be decided and agreed upon between the
Vocation Office and the seminarian.
