REQUIREMENTS & INFORMATION
1. You are to attach 2 recent photographs of yourself (no more than 3 months old, of post card size and in colour):

1 close-up facial shot
1 full-length shot.
One copy must be certified by a Justice of Peace or Commissioner of Oaths.
Photos are to be clear glamour shots and if possible to be professionally taken.

2. You are to attach a notarised copy of your birth certificate or first page of passport to the completed application form.
3. Ensure that any additional pages used are to be clearly labelled and attached to the form.
4. This form should only be submitted after you have carefully read and understood the Terms and Conditions and have
signed the declarations on the previous page. A copy of the Terms and Conditions can be obtained from
www.hibiscusfiji.com and you must retain a personal copy.
5. All information provided in this application form is kept strictly confidential.
6. Only short-listed applicants will be notified through telephone or e-mail of their audition dates.
7. The closing date for ALL applications is Saturday, April 8th 2017 at 4pm.
