This form is to be completed by the student to verify information reported on the Free Application for Federal
Student Aid (FAFSA). Our records indicate that you do not currently have a bachelor’s degree but on your FAFSA
you indicated you will have your first bachelor’s degree by July 1, 2016. Incorrectly indicating you have already
obtained a bachelor’s degree could potentially lead to a reduction in Financial Aid eligibility.
Select the one option below that applies to you.
I will not have my 1st Bachelor’s degree before July 1, 2016.
Choose this option if you have not attended a college or university before or if you are in college right now but will
not have a bachelor’s degree by the start of the 2016-2017 academic year.
I have or will have my 1st Bachelor’s degree before July 1, 2016.
Choose this option if you attended CU Boulder or another institution and you graduated with a bachelor’s degree.
Please note: If you have attended another institution, the Admissions office may request official college transcripts.
I will not have my 1st Bachelor’s degree before July 1, 2016 but I will have my 1st Bachelor’s degree by
the start of the 2016-2017 academic year.
Choose this option if you are graduating in the summer semester at CU-Boulder or another institution and you will
receive your bachelor’s degree at that time.
