3. Offer expires 12/31/16. Requests must be postmarked within 60 days of the last purchase date

Refund request may not be assigned, transferred or sold. Use of multiple addresses or P.O. boxes to obtain
additional refunds is fraud. Fraudulent submissions may result in prosecution. Please allow 4−6 weeks from
receipt of required documentation for check processing. Only original Official Mail-In Refund Form, cash
register receipts and UPCs will be honored. No photocopies, faxes, mechanically reproduced, forged or
altered Official Mail-In Refund Forms, cash register receipts or UPCs will be allowed. Keep copies of all
materials submitted; originals become the property of Sponsor and will not be returned. Sponsor is not
responsible for delayed, postage-due, illegible, mutilated, late, lost, incomplete, stolen or misdirected
requests. Duplicate requests and requests that do not strictly comply with these terms in Sponsor’s discretion
will be rejected and returned. Refund amount is limited as described above. This offer may not be
