A set normally consists of four or five vials of graduated dilutions as prescribed. There is a $8.00 surcharge for all patient billing unless

Sterility Assurance
HollisterStier Allergy’s extracts are manufactured in a cGMP environment, and they undergo a 14-day sterility
testing to ensure safety.
Allergenic Products Return Goods Policy
1. All sterile products (including allergenic extracts) cannot be returned for credit or exchange. If the product was
shipped in error by HollisterStier or received damaged, contact the HollisterStier customer service department
within seven days of receipt of merchandise. We will investigate and contact you within five working days.
2. Phone orders are read back to the customer to ensure accuracy.
3. Warehouse items may be considered for return by contacting our customer service department. Items approved
for return will be subject to a 15% restocking charge.
4. Cutomer service will provide a return authorization form that must accompany all returned products. If a
product is returned without an authorization form, it will be destroyed and no credit will be issued.
5. All returned allergenic extracts must be discarded. Unopened, undamanged warehouse items can be restocked.
6. The customer is responsible for all return shipping charges (except damaged goods and HollisterStier errors).
7. No credit will be issued for customer ordering errors (including duplicate orders).
8. HollisterStier Allergy reserves the right to inspect all returned product before issuing a credit. Ancillary products
must be returned in unopened, undamaged cases to qualify for a credit.
