Job Description
Job Summary:
Pomona Community Health Center, a growing FQHC Community Clinic to residents of the San Gabriel
Valley area, is seeking a qualified Family Practice Physician. The candidate for the position will provide
comprehensive primary and urgent care to an ethnically and socially diverse population in accordance
with the values, beliefs and policies of PCHC.

Essential Duties and Responsibilities:















Collect, record, and maintain patient information, such as medical histories, reports, and
examination results.
Conducting physical examinations.
Order and interpret the results of laboratory, x-ray and other tests.
Determine and implement appropriate courses of treatment.
Provide treatment plans and prescribe medications for issues/diseases.
Monitor results of treatment; changing as necessary.
Maintain accurate, complete records that comply with contracts and/or PCHC policies.
Advise patients and community members concerning diet, activity, hygiene and disease
prevention.
Confer with student and resident physicians and other professional and support staff regarding
care and treatment of patients and assists/mentors them in management of illness: serves as a
resource to resolve patient/family concerns.
Monitor and ensures compliance with clinical evidence-based guidelines for adult and pediatric
health care.
Having the ability to develop positive rapport with patients and families to foster the
physician/patient relationship.
Perform related duties as required.
Immunize patients from communicable diseases.
Advise patients concerning diet, hygiene, and methods for prevention of disease.
Refer patients to medical specialist or other practitioner for specialized treatment.

Actively participate in Quality Management Program, to include but not limited to peer review
and utilization review.
Actively participate in the Infection Control Program, Safety Program, Emergency Preparedness
Program, and Risk Management Program.
Qualifications/Requirements:







MD/DO from an accredited school
Experience with NextGen Electronic Medical Records system; preferred
Minimum 2 years’ experience.
Completed residency training in Family Practice.
Board Certified or Board Eligible for recent grads.
Bilingual in Spanish/Vietnamese preferred.
