WEST VIRGINIA SCHOOL OF OSTEOPATHIC MEDICINE Office of Human Resources New Employee Orientation Department and Mentor Checklist Employee: Position Title: Start Date: Supervisor: Mentor: Introductions and Tours Meet with department supervisor; tour the employee’s work space and immediate work area Meet with department employees and next level supervisor Introduce employee to his or her mentor - The mentor is to assist the employee in their orientation, introduce them to other employees, provide a campus tour, and act as a general resource in responding to questions. Department Orientation – Meeting with Direct Supervisor Explain your personal management style and your role as a supervisor; this includes Problem Solving - you are there to help when a problem arises or a question need to be answered. Review of general functions of department and its relationship to total organization. Discussion of department policies and procedures, which includes the following: Discussion of work schedule; rest periods; lunch break; overtime; time sheets; etc. Department attendance procedures - How, when and who to notify for all types of leave. Telephone, email, and internet use within the department, both personal and professional Office organization and resources (files, supplies, etc) Department dress code and personal conduct standards Confidentiality and the rules related to the protection of student information, graduates and employee file pertinent to the position. Probationary Period (for Classified Employees Only) - Review of performance evaluation system, areas of evaluation and what supervisor will expect and look for during this period. Provide employee with the appropriate keys Have a photo taken for the employee’s identification card and website directory (ext. 6209) Employee to provide the Maintenance Department (ext. 6812) with employee’s name, driver's license number, state where issued and expiration date in order to allow use of state vehicles. Mentor Orientation Mentors will work with new employees to cover the following: Provide employee with a campus tour and introductions Orient the new employee to the WVSOM community and culture Provide basic information regarding WVSOM and the department Review the basics of the department equipment use such as phones, computer, ID card, copiers, fax machines, mail drop off, etc. Employee Trainings The supervisor and employee should arrange for any training as necessary including: Information Technology Training for email and network access, and schedule for additional trainings pertinent to the department and position (ext. 6246) Title IX Training (link to be provided to employee) Legiant Leave Training (ext. 6221) Family Education Rights and Privacy Act, FERPA, Training (link to be provided to employee) Accounts Payable Office Orientation including travel regulations, reimbursement procedures, and Purchasing Card rules/regulations (ext. 6221) Orientation on purchasing procedures for large expenses (ext. 6249) Banner Self Service Budget Query Training when applicable to the position (ext. 6315) Receiving and Inventory orientation (ext. 6359) Osteopathic Principles & Practices Orientation (ext. 6335) Curriculum orientation with Preclinical Education (ext. 6301) Media Services Orientation (ext. 6897) Review and discussion of the WVSOM Statewide Campus (ext. 6853) Vice President for Administration and External Relations Orientation (Non-Classified and Faculty only; ext. 6200) Upon completion of this checklist, please sign and return to the Office of Human Resource.
