Things that parents should take into consideration:
Some children may need to be vaccinated if they missed
vaccine doses during their pre-school years of life.
Make sure your child has taken all required vaccines
before starting school (childhood vaccination).
Some vaccines are recommended during pre-teen and
teenage years (school years).
Follow the vaccination requirement based on your child’s
grade.
Some vaccine-preventable diseases are still common and
unvaccinated children are at risk of infection.
If your child has missed any vaccination before or after
reaching school age, make sure to communicate with the
school clinic.
If the vaccination rate in the community decreases, the
risk of infectious disease outbreak increases.
Vaccination is one of the most successful and
cost-effective health interventions ever introduced.
Vaccination programs in many countries have
significantly reduced number of cases and deaths due
to infectious diseases worldwide. Some diseases, such
as smallpox, have been completely eradicated, thanks
to vaccination programs.
Complete the consent form, sign and return it to the
school.
If you refuse the vaccination, the reasons should be
mentioned clearly on the form and a medical report or
previous immunization record must be provided to assess
the vaccination status of your child. Exemptions are
subject to approval from the service provider.
Vaccines are safe an effective way to prevent your child
from getting ill. Before licensing any vaccine, it is tested
extensively for many years around the world. Once a
vaccine is in use, the Health Authority – Abu Dhabi
along with other national and international authorities
are committed to assuring the safety of vaccines
through strict monitoring systems.

‫ﻗﺪ ﺗﺰداد ﺣﺎﻻت ا‪¢‬ﺻﺎﺑﺔ ﺑﺎﻣﺮاض اﻟﻤﻌﺪﻳﺔ إذا اﻧﺨﻔﻀﺖ ﻣﻌﺪﻻت‬
‫اﻟﺘﻄﻌﻴﻢ ﻓﻲ اﻟﻤﺠﺘﻤﻊ‪ ،‬أي ﻋﻨﺪﻣﺎ ﻻ ﻳﺘﻢ أﺧﺬ اﻟﻠﻘﺎح ﻣﻦ ﻗﺒﻞ‬
‫ﺟﻤﻴﻊ اﻟﻔﺌﺎت اﻟﻤﺴﺘﻬﺪﻓﺔ‪.‬‬
‫ﻓﻲ ﺣﺎﻟﺔ ﻋﺪم اﻟﻤﻮاﻓﻘﺔ ﻋﻠﻰ أﺧﺬ اﻟﺘﻄﻌﻴﻢ‪ ،‬اﻟﺮﺟﺎء ذﻛﺮ ﻣﻮاﻧﻊ‬
‫اﻟﺘﻄﻌﻴﻢ وإرﻓﺎق ﺻﻮرة ﻋﻦ اﻟﺘﻘﺮﻳﺮ اﻟﻄﺒﻲ أو ﺷﻬﺎدة اﻟﺘﻄﻌﻴﻢ‬
‫اﻟﻤﺴﺒﻘﺔ‪ ،‬وذﻟﻚ ﻟﺪراﺳﺔ اﻟﺤﺎﻟﺔ واﻟﻤﻮاﻓﻘﺔ ﻋﻠﻰ اﻻﺳﺘﺜﻨﺎء ﻣﻦ‬
‫اﻟﺘﻄﻌﻴﻢ ﻣﻦ ﻗﺒﻞ اﻟﺠﻬﺔ اﻟﺼﺤﻴﺔ اﻟﻤﻘﺪﻣﺔ ﻟﻠﺨﺪﻣﺔ‪.‬‬
‫ﺗﻌﺘﺒﺮ اﻟﺘﻄﻌﻴﻤﺎت واﺣﺪة ﻣﻦ أﻛﺜﺮ اﻟﺘﺪﺧﻼت اﻟﺼﺤﻴﺔ ﻧﺠﺎﺣ•‬

‫ا‪°‬ﺻﺎﺑﺔ ﺑﺎžﻣﺮاض‪ ،‬ﺣﻴﺚ أﻧﻬﺎ ﺗﺨﻀﻊ إﻟﻰ اﺧﺘﺒﺎرات ﻋﻠﻰ ﻧﻄﺎق‬
‫واﺳﻊ ﻟﻀﻤﺎن ﺟﻮدﺗﻬﺎ ﻗﺒﻞ ﺗﺮﺧﻴﺼﻬﺎ ﺣﻮل اﻟﻌﺎﻟﻢ‪ .‬وﻋﻨﺪ‬
