Agreed Weekly Rate: __________________________________________________________
Please note that fees are payable one month in advance and payment can be made by cheque or cash. Payment is
due irrespective of whether or not your child is present. This is necessary so that we can cover staff wages, who
will still be in attendance at the nursery.
No fees are payable when nursery is closed i.e. Bank Holidays and the period between
Christmas and New Year
One month written notice of termination must be given if your child will be leaving the nursery. Failure to do so
will result in the contracted fees being payable.
Please note: the £20-00 registration fee is to secure a place for your child on the waiting list. It is nonrefundable and is not deducted from the first month’s fees.

If the named parents or guardian overleaf are not in a position to collect a child, a named and identified agent may
act in your capacity.
In the interests of safety for your child it will be necessary for you to bring your agent in to the nursery to be
formally introduced to the staff (so that they can be recognised in the future). We also ask that the agent has a
password to give staff if asked, as added security and to aid recognition.
In addition we would appreciate prior notification that your agent will be collecting your child from nursery.

Parental Consent Form: Use of photographic images
As part of Documenting and Recording your child’s learning journey, Busy bears will take photographs
and video recordings your child at our nursery for ‘educational use’. This will include displays or using the
images on project boards, in nursery newsletters, or taking pictures of children for historical purposes
such as class photographs and for use on our website. Photos will also be used to build on your child’s
Magic Moments Scrap Book and at times it may be possible that a group image is used. Therefore the
Data Protection Act 1998 requires that we obtain your permission before we photograph or make use of
any recordings or pictures of your child when used for promotional purposes.
I have read and understood the Conditions of Use and have stated the purpose(s) for
which my child’s image may be used.

Further Photographic Consent
Given the outstanding nature of Busy Bears Children’s Day Nursery we are often approached by Durham
County Early Years Team, Sure Start Officers, and Ofsted inspectors for permission to take
photographic images and video footage as part of their on going training for practitioners. These images
will only be used for educational purposes as examples of effective practice.
We therefore seek further consent for your child to feature in any of this photographic material.
