The father, mother or tutor want that the underage active user (whose personal data is given at the top of tis
page) can sign up to 360ºextrem Club at the installations of Center Freestyle Indoor 360ºEXTREM. We accept
the conditions of use (that we can read on the back). And to make the understand, we sign this copy in Andorra la Vella
with the date that is indicate at the bottom as a consent proof./.

Regulations for the use of 360º EXTREM Indoor Freestyle Centre
· The active user has decided to participate, of his/her own free will, in the leisure and entertainment activities offred and
organised by the 360ºEXTREM Indoor Freestyle Centre, wich are carried out in the facilities in Avinguda Santa Coloma nº115, de
Andorra la Vella.
· the active user shall be able to use these installations during the period of the activity.
· Signatory of this documents is fully aware of the risks involved in practising freestyle in any of its modalities (skate, roller,
scooter, ski, BMX, snowboard, etc...), including those inherent in using the ramps and other typical facilities when practising the
various freestyle disciplines. He/she is also aware that freestyle is an extreme sport and, therefore, potentially dangerous and
that it implies risks of a physical and material nature, in which accidents can occur that can lead to death as well as injuries to
the person practising it and/or to third parties.
· Signatory of this document declare that in knowledge of the risk of freestyle activity, He/she herebys state that he/she is in an
ideal condition, both physically as well as psychically, and that he/she has a sufficient level of knowledge of the technique to
be able to practice it in the 360º EXTREM Indoor Freestyle Centre facilities, aware of the risk that its use involves and, therefore
fully accept all the responsibility derived from its use, whether in the case of death or injuries to myself or to third parties, as well
as damages to the facilities and my own material or that of third parties.
· Signatory of this document declare that he/she fully familiar with the conditions of use of the 360º EXTREM Indoor Freestyle
Centre facilities contained in its regulations for use of which he/she has been provided with a copy, that he/she has read and
that he/ she accept, in particular with regard to safety measures, regulations for use and behaviour and practices that are
completely forbidden. He/she guarantees to comply with them strictly and to use the facilities responsibly, as well as to comply
with the instructions given by the staff and the signalling in the centre. He/she accept the fact that non-compliance may lead to
his/her expulsion from the centre, however long the duration of his/her membership may be, without the right to any refund for
this reason and without affecting his/her obligation to compensate for any damages he/she might have caused to anyone or to
the facilities, whether fraudulently or deliberately.
· The active user will use the 360º EXTREM Indoor Freestyle Centre facilities under his/her own exclusive responsibility, waiving
the right to take out any action, be it criminal, civil, administrative or of any other nature, including out of court, against the
company that runs the Centre, as a result of possible injuries and/or damages that he/she might suffer as a result of practising
freestyle in this centre and/or, in general, the use of these facilities.
· Father, mother or tutor implicate to make the underage understand and comply with the regulations for use of the facilities of
the Centre, as well as the instructions and signalling on the use of the equipment and material
· The signatory of this document declare that he/she is familiar with the fact:
- That his/her personal data contained in this document, as well as that which he/she may provide or may be obtained
by the company running the centre by any means during his/her membership at the 360º EXTREM Indoor Freestyle Centre will be
incorporated into a computer file belonging to the company running the Centre, to be processed. He/she particularly give
his/her consents so that the company running the centre may take images of me and that therefore, he/she may be photographed or recorded while participating in activities in the facilities at the centre.
- That the personal data obtained by the means indicated, will be processed in keeping with Law 15/2003, of 18th
December, on the protection of personal data and that suitable measures of security and confidentiality shall be applied to this
data.
- That he/she has the right, with regard to the data obtained as mentioned, to exercise the rights recognised in Law
15/2003, of 18th December, on personal data protection, especially the right to access, rectify or cancel data or oppose it, if
relevant, as well as to revoke my consent for ceding data.
- That he/she expresslys authorise the company running the 360º EXTREM Indoor Freestyle Centre to:
a) Send me information by any means, including electronic means, concerning the promotion of the centre and the
activities that are carried out in it.
b) Give up my personal data to companies in the business group to which the company running the centre belongs
in order to be informed about their products, services and commercial promotions, through any kind of communication, including
electronic means.
c) That the material obtained through photographs and recordings, as mentioned above:
- May be used for the purpose of marketing or promoting the centre, with the right to revoke this authorisation at
any given time, by writing to the company running the centre, in the form and at the address indicated above, for carrying out
the right of cancelling, opposing and revoking their consent for the cession of personal data.
- May be used as evidence in a lawsuit.
d) Process and segment this data, applying the relevant techniques, with the aim of optimising the promotion of the
centre and of the activities carried out in it.
