Uploading Artifacts
1. 2. Login to mylearningplan.com From your main Learning Plan view, under the category ACTION REQUIRED, find your Documentation Log. Next to ACTION, click on the blue hot link to open your form. When you click on the link, the log will open in a new tab or window. You may now view, complete, or edit your information. 3. 4. 5. 6. To upload an artifact, look for the ADD AN ARTIFACT button under the “Directions” box. ALL artifacts are uploaded from this location. Click on the ADD AN ARTIFACT button. Complete the information fields for your artifact. Ø
Name -­‐ Give your artifact file a name Ø
Description – Describe your artifact for your evaluator. What is it? What is the context? Ø
Types – What kind of artifact is this? Check the best type to describe your artifact. Ø
Categories – What category does this artifact fit? Check the best category for your artifact. Ø
Alignment & Criteria – o
First, choose a rubric. Click on the down arrow to drop-­‐down the standards. Choose the one to which your artifact aligns. o
Second, in the next field to the right, click on the down arrow and check the corresponding standard criteria box. o
Third, click the ADD CRITERIA button. You should see the rubric and criteria appear in the box below. Ø
7. File -­‐ Locate your file and click the upload button (blue “up” arrow) Click SAVE 8. REFRESH your screen to see your artifact listed below under the corresponding standard. 9. Provide a summary/reflection of your artifact in the text box. This is an opportunity for you to summarize your artifact and why/how it shows your exceptional skill in this standard. When your artifact log is complete, check the boxes of examples that fit your artifacts for each standard. This gives your evaluator an indication of the kinds of artifacts included for each standard. Specific artifact examples are given under some standards, but these are only examples – not requirements. 10. Be sure to SAVE your form by clicking the SAVE button on the bottom of the form. Click SUBMIT only when log is complete and ready to be reviewed by your evaluator. 11. To close out of the form and return to main page, close this tab and click on the My Learning Plan tab in your browser to return to your main Learning Plan view. 12. To close out of the program, click the LOGOUT button on the main Learning Plan view located in the upper right hand corner just above the orange Learning Plan tab. EP Virtual Support Center
