(IDFPR) announced today that the Acting Director of the Division of Professional Regulation,
Daniel E. Bluthardt, signed the following disciplinary actions taken during the month of

ACCOUNTANT
Jacqueline A. Black, Ingleside – certified public accountant license (065-020919)
indefinitely suspended for a minimum of one year, followed by probation for two years.

issuing deficient surveys and Boyes land surveyor license (035-002519) placed on probation for
three years and fined $3,000 for practicing on a revoked license and aiding an unregistered firm
in Illinois to provide land surveying services and to issue deficient surveys.

suspended for violating his Agreement of Care, Counseling and Treatment by testing positive for
alcohol.
Michael Deaton, Champaign – pharmacy technician license (049-150042) indefinitely
suspended after making a misrepresentation to the Division for the purpose of obtaining a license
by failing to indicate he was convicted of DWI.
Danielle L. Groh, South Elgin – pharmacy technician license (049-144432) indefinitely
suspended for diverting various hydrocodone combinations, all schedule III controlled
substances from her employer and providing them to an acquaintance.
Carl Hunckler, Plainfield – pharmacy technician license (049-151285) indefinitely
suspended for diverting various schedule III and IV controlled substances for self-use.
Jacqueline Junivall, Danville – pharmacy technician license (049-078243) reprimanded
after failing to renew her license from April 2001 through April 2005.

Timothy King, Rockford – pharmacy technician license (049app113842) issued and
placed on indefinite probation for a minimum of two years after having numerous convictions for
speeding, DUI and driving with a suspended license from August 2000 through December 2004.
Tara M. Parnell, Springfield – pharmacy technician license (049-125811) indefinitely
suspended after pleading guilty in Sangamon County Circuit Court of possession of a controlled
substance and was convicted of a class IV felony.
Ricardo Pena, Chicago – pharmacy technician license (049app155973) issued and
placed on probation for one year after being convicted for manufacture or delivery of
unauthorized schedule I and II controlled substances.
Santosh Soparawala, Evergreen – pharmacy technician license (049-128052) placed on
probation for two years after being arrested and charged with three counts of burglary for
breaking into a laboratory and attempting to steal computer equipment from the University of
Illinois at Chicago.
Kenneth Behr, South Bend, IN – pharmacist license (051-037616) suspended for 30
days immediately followed by indefinite probation for a minimum of three years after being
disciplined in the states of Michigan and Indiana.
Robert P. Bray Jr., Oak Lawn – pharmacist license (051-037756) summarily
suspended for habitual or excessive use of drugs resulting in the inability to practice with
reasonable judgment, skill or safety, unauthorized possession of controlled substances by
diversion, repetitiously dispensing prescription drugs without receiving a written or oral
prescription and dishonorable, unethical and unprofessional conduct.
Matthew J. Carlisle, St. Charles, MO – pharmacist license (051-035160) placed on
indefinite probation for a minimum of three years after being disciplined in the state of Missouri.
Community Pharmacy Services Inc. and Salem E. Najjar, Chicago – pharmacy
license (054-014839) placed on probation for one year and pharmacist license (051-031095)
placed on probation for one year and fined $2,000 for allowing prescription medications to be
dispensed without a pharmacist on duty in the pharmacy, failing to post the required signage
whenever a pharmacist is not present and the pharmacy is open to the public and failing to have
the pharmacy open for the required forty hours per week minimum.
Aaron Hambrick, Chicago – pharmacist license (051-288960) placed on definite
probation to terminate November 9, 2009 after diverting various controlled substances for self
use from his employer.
Matthew Kolb, Chicago – pharmacist license (051-288637) placed suspension for 30
days immediately followed by indefinite probation for a minimum of five years after being found
to be in possession of three tablets of Carisoprodol, a prescription medication, without a
physician’s authorization for self-use and a vial of liquid Methadone, a schedule II controlled
substance, which was pursuant to a valid prescription for opiate dependency.

Mary C. Mulder, Wauconda – pharmacist license (051-027321) suspended for 14 days,
immediately followed by probation for two years after repeatedly self-dispensing prescription
medications, including Propoxyphene Napsylate/APAP, a schedule IV controlled substance,
without receiving a written or oral prescription.
Lisa M. Russell, Houston, TX – pharmacist license (051app28148) issued and placed on
probation for one year after being disciplined in the state of Texas.
Jeffrey K. Connell, Goreville – pharmacist license (051-030935) placed on probation
for 30 months after causing false statement to be made to the Illinois Department of Public Aid
and misbranding drugs.
Lea Anne Croiser, Grand Island, NE – pharmacist license (051-035487) placed on
indefinite probation for a minimum of four years after being disciplined in the state of Nebraska.
Sharon Paloucek, Western Springs – pharmacist license (051-037453) summarily
suspended due to dishonorable, unethical and unprofessional conduct of a character likely to
harm the public after being arrested for the unlawful use of a weapon.
Jodi Sheahan, Wentzville, MO – pharmacist license (051-039889) reprimanded after
being disciplined in the state of Missouri.
Maureen Visona, Crystal Lake – pharmacist license (051-033394) reprimanded after
diverting from her employer three tablets of hydrocodone/APAP 2/500, a schedule III controlled
substance, for self use.
Richard Wahlstrom, Chicago – pharmacist license (051-031184) reprimanded for
failing to maintain proper recordkeeping requirements as a pharmacist-in-charge and failing to
provide pharmaceutical services to the public for the 40 hour per-week minimum requirement.
Carnegie Sargents Pharmacy, Chicago and Mark Paley, Highland Park – pharmacy
license (054-011275) reprimanded and fined $2,500 and pharmacist license (051-035351)
reprimanded for dispensing a medication that was not FDA-approved to a patient that had been
purchased from a wholesale drug distributor located in Great Britain which was not licensed in
the state of Illinois.
Osco Drug, Chicago – pharmacy license (054-003928) reprimanded and fined $7,000
for failing to have a pharmacist-in-charge from February 20, 2004 through September 10, 2004.
PHYSICAL THERAPY
Kurtis L. Miller, Milwaukee, WI – physical therapy assistant license (160-002041)
reprimanded after being disciplined in the state of Wisconsin.

Benny Greenspan, Palos Heights – podiatrist license (016-004117) ordered to cease and desist
the practice as a podiatrist with a non-renewed license until such time his license is restored to an
active status.
Timothy Horak, Homewood – podiatrist license (016-004735) placed on probation for
two years and fined $2,500 for overcharging a patient.
MASSAGE THERAPIST
John Maley, Chicago Heights – massage therapist license (227app4383) issued and
placed on indefinite probation for a minimum of five years due to criminal convictions.
PSYCHOLOGY
Russell Morgan, Plainfield – clinical psychologist license (071-005241) revoked due to
unethical, unauthorized or unprofessional conduct.
PROFESSIONAL COUNSELORS
Daniel J. Becco, Chicago – professional counselor license (189-006880) indefinitely
suspended for a minimum of six months for exceeding the scope of his licensure by engaging in
independent private practice.
ROOFING CONTRACTORS
Affordable Home First Contractors, Chicago – roofing contractor license (104012663) indefinitely suspended for accepting $1,348 from a complainant to re-roof a dwelling.
Respondent removed existing roof then notified complainant that they were canceling the
contract and leaving roof unfinished.
Harold Smithee Construction, O’Fallon – roofing contractor license (104-009999)
indefinitely suspended for practicing on a non-renewed license, accepting money and failing to
do the roofing job and two felony convictions.
Eatmon Roofing, Washington – roofing contractor license (104-000698) placed in
refuse to renew status until respondent can demonstrate that it is fully in compliance with the
laws of the state of Illinois and is competent to perform roofing services to the satisfaction of the
department.
