Specialty Drug Pharmacy Program Specialty medications are generally administered through injection or infusion, and treat rare or complex diseases that require clinical management. These drugs are extremely expensive and may require special handling, administration or monitoring. Plains All American utilizes the OptumRx Specialty Drug Program. Medical Reimbursement Plan - This plan is for payroll deductions for predictable health, dental, vision, and hearing expenses not covered by other benefit plans up to $2,500. A Healthcare Spending Card is provided for use at point of sale. Dependent Care Plan – This plan is for payroll deductions for predictable dependent care expenses with a maximum of $5,000 (if you are married filing a joint return or you are head of a household) or $2,500 (if you are married filing separate returns). LIFE AND AD&D INSURANCE Basic Life and AD&D Plains All American provides Basic Life Insurance coverage equal to two times your annual salary. This coverage is subject to a maximum of $140,000 for truck drivers and $500,000 for all other employees and is paid for by the company. The Company also provides AD&D coverage equal to five times your annual salary to a maximum of $350,000 for truck drivers and $500,000 for all other employees. Voluntary Life Plains All American offers Voluntary Life Insurance. You can receive up to $500,000 or five times your annual salary (whichever is less) for employee coverage. Spousal coverage amount can be elected up to $250,000, and child up to $10,000. Voluntary AD&D Plains All American offers Voluntary Accidental Death & Dismemberment Insurance. You can receive up to $500,000 for employee coverage. Spouse and child coverage is based on a percentage of the employee’s amount (10%-60%) dependent upon who is covered. Short Term Disability Holidays The Company provides Short Term Disability Insurance at no cost to you. This benefit provides 100% of your base weekly earnings for six weeks, then 60% of base earnings. This benefit duration is 180 days for truck drivers and 90 days for all other employees. New Year’s Day Long Term Disability The Company provides Long Term Disability Insurance at no cost to you. This benefit provides 60% of your monthly earnings to a maximum of $3,500 per month after a waiting period of 180 days for truck drivers and $10,000 per month after a waiting period of 90 days for all other employees. Employee Assistance Program – Care24 Care24 provides resources to assist you with the resolution of challenges involving health, personal, family, financial, legal and work-related issues. Care24 registered nurses and counselors are available 24 hours a day at 1.888.887.4114. 401(k) On your first day of employment you will be eligible to participate in the Plains All American 401(k) Plan. You may make pre-tax contributions of 1%50% of gross pay not to exceed the IRS maximum ($18,000 for 2015). The Company may match your contributions with cash up to an aggregate of 6%. Your before-tax account and the company match account are both 100% vested at all times. Rollover contributions may be accepted from other qualified plans upon employment. Catch-up contributions up to $6,000 as provided by the IRS are allowed for employees age 50 and over or who will be age 50 by year end and who are contributing the maximum before tax contribution. Employees may carry over up to 2 weeks of vacation into the following year. Sick Leave Full-time employees receive 80 hours of sick leave per year. Drivers receive 100 hours per year. For new employees sick time hours are pro-rated based on hire date. Credit Union As a Plains All American employee, you are eligible to participate in the Peoples Trust Federal Credit Union. Educational Reimbursement With management approval, employees who are continuing their education may receive up to 100% reimbursement for tuition and books after satisfactorily completing courses leading to a degree (up to $5,250).
