This rebate form is eligible for qualified installations and/or services performed between January 1, 2016, and December 31, 2016; based on approval of the Minnesota
Department of Commerce Division of Energy Resources (DER) and available dollars. All forms must be postmarked within 30 days of installation and/or service to be
considered eligible for the rebate. Please allow 8 - 10 weeks to receive the rebate(s).
CONTRACTOR INSTRUCTIONS
Verify that customer’s electric utility at the installation address is Minnesota Power or a participating municipal utility.
Contractor is responsible for submitting rebate forms and required paperwork to the address below. See page 4 for submission instructions.

PRE-INSTALLATION SUGGESTIONS
Many airflow issues are due to lack of maintenance. Airflow restrictions will decrease efficiency and increase operating noise. Some of the following suggestions
may lead to additional service requirements and cost.
■■ Open and clear all registers and grilles.
■■ Inspect the filter(s). Recommend cleaning or replacement if visibly dirty.
■■ Inspect the blower wheel. Recommend cleaning if visibly dirty.
■■ Inspect the evaporator coil if accessible. Recommend cleaning if visibly dirty.
■■ Measure the TESP at the highest airflow demand. If TESP is above .8, diagnose and repair the airflow issue.
