mail-in rebate.
Customers may only receive one rebate per piece of qualifying equipment. This offer
is available to customers of participating municipal utilities listed on the back of form.
Equipment must be installed in facilities that are served electrically by the participating
utility. Utility representatives may conduct an inspection to survey any installed equipment.

This ENERGY STAR rebate offer is good for the
purchase of an ENERGY STAR qualified product
listed at left. Restrictions apply as noted. This offer is
available to customers of participating municipal
utilities listed on the back of this form. Customers may
only receive one incentive per piece of qualifying
equipment. To receive your rebate, please fill out this
form completely and mail it to your participating utility
(listed on the back) with a copy of your sales receipt
and recycling receipt, if applicable. Incomplete
information will delay or disqualify your rebate. Rebate
applications must be postmarked within 30 days of
purchase.
This rebate program is reviewed and authorized each
year for incentives that are available from January 1
through December 31. Due to limited funding, this
rebate offer is available on a first-come, first-served
basis, and can be changed or withdrawn at any time
without notice. The utilities do not warrant the
performance of installed equipment, expressly or
implicitly. Offer is subject to change without prior
notice. Void if copied or transferred or where
prohibited by law. One rebate per product purchased.
