MPSD PARENT/GUARDIAN AND STUDENT NOTEBOOK COMPUTER OR “BRING YOUR
OWN TECHNOLOGY” (BYOT) RESPONSIBILITY CONTRACT AND CONSENT FORM
Manitowoc Public School District is pleased to provide students with a notebook computer to facilitate learning and
communication. All notebook computers, related equipment and accessories are Manitowoc Public School District
property. The use of a District notebook computer is a privilege extended to students for use inside and outside the
school facility. The notebook computers are provided for use as a productivity tool for education-related purposes
only. To participate in this program, students and their Parent/Guardian are required to sign this ‘MPSD
Parent/Guardian and Notebook Computer or Bring Your Own Technology (BYOT) Responsibility Contract and
Consent Form’ in the spaces provided below and to comply with all of the following terms as a condition of using
Manitowoc Public School District’s notebook computer.

Responsible Use Policy
1. The Parent/Guardian and Student have been provided with and reviewed the District’s Student Network and
Internet Acceptable Use and Safety Policy (Policy 7540.03) and understand and agree to comply with the
Acceptable Use and Safety Policy as a condition of using the District’s notebook computers. The terms of the
Acceptable Use and Safety Policy are incorporated by reference in this Contract and Consent.
2. Student use of “proxy” websites to access sites blocked by the District content filters is not allowed as a part of
the Acceptable Use and Safety Policy. Student use of “proxy” websites is a deliberate effort to circumvent
content filtering software. Students using “proxy” websites for any reason, or otherwise purposely bypassing
District content filters will be disciplined accordingly. This can include but is not limited to revocation of network
access privileges, confiscation of student notebook computer and other school discipline measures.
Ownership & Responsibilities
3. Parent/Guardian and Student understand that the notebook computer is District property and that access to and
use of the District’s notebook computers inside and outside of the school facility is a privilege. Parent/Guardian
and Student agree to surrender the notebook computer to the District immediately upon the request of District
personnel.
4. Parent/Guardian and Student understand and agree that the District does not warrant the fitness or performance
of District notebook computers and that the District shall not be responsible for the loss of any document, file or
other information saved on the notebook computer. The District shall further not be responsible for any items
left in the notebook computer case, such as textbooks, homework, documents, thumb drives, etc.
5. Parent/Guardian and Student understand that because the notebook computers are public property of the District,
appropriate District staff may read or look at any item on District notebook computers and review computer
activity logs and Internet history on notebook computers to ensure compliance with the Acceptable Use and
Safety Policy. Parent/Guardian and Student will not alter or delete the computer activity logs or Internet history
file. The student, members of the student’s family or any other user have no privacy interest whatsoever in the
notebook computer and any media stored on the notebook computer.
Parent & Student Responsibilities
6. Parent/Guardian and Student understand and agree that the District has taken all reasonable and prudent measures
to ensure that students who use school notebook computers to access and use the Internet will use the notebook
computers in a safe and appropriate way while the computers are being used on school premises. Parent/Guardian
and Student understand and agree that the parent/guardian is solely and exclusively responsible for monitoring
the student’s use of District notebook computers and access to the Internet outside the school environment and
to ensure that students access the Internet and use the notebook computers in a safe and appropriate manner.
7. Parent/Guardian and Student understand that Internet filtering software is applicable for notebook computers
when accessing the Internet on school grounds and the MPSD Wi-Fi. The Parent/Guardian and Student are
responsible for following the Acceptable Use and Safety Policy even when using District notebook computers to
access unfiltered Internet providers.

8. Parent/Guardian and Student agree to assume full responsibility to the District for the theft, loss or damage to
notebook computers and computer equipment provided by Manitowoc Public School District which is beyond
normal wear and tear. If District notebook computers and/or computer equipment are lost or stolen or sustain
damage beyond repair, the Parent/Guardian and Student shall be responsible for replacement cost based on the
year of operation (see chart below). The Parent/Guardian and Student understand that all lost or stolen computer
equipment should be reported to police for investigation and possible prosecution and agree to cooperate with
any investigation. The replacement cost schedule is as follows:

9. Parent/Guardian and Student agree to indemnify, defend and hold the District, its employees, agents and
representatives harmless from any and all claims relating to or arising out of the Parent/Guardian and/or Student’s
use of the notebook computer regardless of whether such claims are caused, in whole or in part, by any act or
omission of the District, its employees, agents or representatives.
10. The students will receive the computers in 6th grade and 9th grade. The 6th grade student will have the computer
from 6th through 8th grade. The 9th grade student will have the computer from 9th through 12th grade. The student
is responsible for the notebook computer throughout the summer. The computer needs to be logged by MPSD
at the end of each year.
11. Students who leave the District for whatever reason, must return their computers to the school within 10 days of
their withdrawal or will owe the full replacement cost.
12. Notebook insurance will be available at $40.00 for the life of the computer (three or four years). There is a
deductible of $50 for each incident. The insurance includes losses for the following:
a. Accidental damage (such as cracked screens)
b. Vandalism
c. Natural disasters
d. Liquid spills
e. Fire and Flood damage
f. Power surge from lightning
Bring Your Own Technology (BYOT)
13. MPSD will now be incorporating the use of such items as notebooks, iPads, netbooks and cell phones with
browsing capabilities for educational purposes only. Similar to other personally owned items, the District is not
liable for the loss, damage, misuse, or theft of personally owned technology devices brought to school.
14. We encourage students to take advantage of the district notebook computer program. However, if you decline
the district notebook computer, you are selecting the BYOT option and will be responsible for supplying a
different device, capable of web browsing and word-processing. Regardless of whether students use district or
personal technology, no one will be left out of the instruction process.
15. Expectations of students regarding BYOT:
a. Students will only use appropriate technology at the discretion of administration and teachers.
b. Students will only use appropriate educational applications on their personal technology device while in
the classroom instructional setting (i.e. no games and/or non-school related tasks and functions).
c. Students are permitted to access only the school’s network through personal technology devices, not
private networks. More information will be provided to students for accessing the available network.
d. Students not following expectations for use of personal technology devices will face school disciplinary
measures and lose the privilege to utilize personal technology devices in school for a period of time
commensurate with the infraction of Internet access. See District’s Acceptable Use and Safety Policy
(Policy 7540.03) for more information.

Signed Agreement
The agreement will be signed once for the duration of the student’s attendance in the Manitowoc Public School
District unless revoked by the District or the parent/guardian.

Optional Notebook Insurance
For a minimal fee plus a deductible, you can protect your notebook computer and ease your mind by knowing that
the notebook will be covered against any accidental damage.
The notebook insurance provides coverage that will protect your notebook computer from accidental damage, liquid
spills, power surge, vandalism, flood, fire and more for the life of the computer with the District (3-4 years) for a low
cost of $40. Be aware that the notebook insurance policy does not cover lost or stolen computers. In addition, there
is a $50 deductible per incident. This means that any repairs or replacement of the notebook will require a payment
of $50 to cover the deductible. The $40 notebook insurance must be purchased at the time of pick-up; there will be
no other opportunity to do so. Please indicate your decision by checking one of these boxes:
