 PERMISSIONS The following has been put in place to safeguard your child and will be adhered to should your child be offered a place at Discovery. A place will be offered on the understanding that you have read, understood and agree with all the permissions in this document. Permission for the staff at Discovery to administer First Aid, including the administering of prescribed drugs, in the event of an emergency. In an emergency, when a parents’ attendance cannot be immediate, it may be necessary to obtain treatment for a child from a doctor or casualty department/ or in the event of a sudden illness or accident, where emergency treatment, including a ny operative treatment and/or administration of a general anaesthetics to my child. The doctors attending to your child needs will of course, decide this. Permission for my child to be taken to a hospital in case of an emergency. Permission for the application of sunscreen lotion/ nappy rash cream, at any time, which is to be provided by the parent and labelled with child’s name. The setting will not take responsibility for any allergic reaction. Permission for the staff at Discovery to escort him/her on outings: i.e. visits to the library/supermarket/park, woods near the settings, forest school sessions / walks around the area and any other outing organized for the children. A high staff/child ratio will be in place to ensure safety. Permission to use the park equipment (climbing frame, swings, roundabouts. Climb tree etc…) outdoor play & climbing equipment at the setting (movable/non-­‐movable). Members of staff will supervise children at all times. Safeguards are in place, however in the case of any accident, procedures will be in line with our policy (accessible via the website or available at any time at the setting) and Ofsted guidelines. Permission for all staff at Discovery to administer medication once medication is in the setting and complies with the policy and procedures. AGREE DISAGREE It is our policy NOT to give children medication at the nursery unless it is for LIFE THREATENING conditions such as asthma, severe allergies etc. A doctor or dentist must prescribe medication. Medication must be clearly labelled with child’s name and dosage 2.

Authorisation from the parents – for each and every medication It is your responsibility to replace medication out of date and inform the setting if medication is no longer needed or if there is a change in the dosage. Should my child develop an allergy or intolerance to any food/material during their time at Discovery, I hereby give permission for this to be entered on the Allergy List and displayed on the classroom wall. Permission for photographs/film clips taken of my child during the normal course of events at the nursery to be used for external publicity and promotion. The nursery regularly takes pictures of the children during the course of events at the nursery for display purposes (and for the restricted gallery available for parents viewing at Parentzone). It is also the custom and practice of most independent nurseries, and of this nursery, to include some photographs or images of the children for external publicity and promotion. These images may be used indefinitely without compensation to you and you waive any rights, now or in the future, to inspect or approve the finished product. You accept that all negatives, positives, prints and digital reproductions will be the property of Discovery a Nursery & Preschool.
