Pursuant to Municipal Code of Chicago §4-60-040(h), the Department of Business Affairs and Consumer
Protection (BACP) and Local Liquor Control Commission has authorized the issuance of a Packaged Goods
Liquor License, at the premises located 1455 W. Taylor St, to the above-named business entity and business
location under the following conditions:
1. Hours of Operation: Monday, Tuesday, and Wednesday-12 Noon to 10 pm. Thursday, Friday, and
Saturday -12 Noon to 11pm. Sunday- 12 Noon to 8 pm.
2. Signage and Window Coverings: The business will not have LED or neon signs of any kind in or
within 10 feet of any openings or on the exterior of the premises. Not more than 15% of any
windows shall be covered with any signage or lettering. Also, there will be no paper signs in
windows.
3. Tobacco Sales: The business owner will not be able to sell the following: Mini-Cigars, Cigarettes,
rolling papers and loose tobacco, other tobacco products (excluding Cigars), single tobacco
products (i.e., 1 Cigar not 1 pack of Cigars) less than $10.00, to be adjusted bi-annually based upon
the November 2014 Consumer Price Index- Urban Wage Earners and Clerical Workers (Chicago All
Items) published by United States Bureau of Labor Statistics.
4. Beer & Wine Sales: The business owner will not sell single servings of beer in containers less than
24 fluid ounces and not less than $4.75 per single serve beer, to be adjusted bi-annually based
upon the November 2014 Consumer Price Index- Urban Wage Earners and Clerical Workers
(Chicago All Items) published by United States Bureau of Labor Statistics. The business will not sell
beer in its original packaging from the distributor at a price of not less than $4.75 per unit. There
will be no sales of Fortified Wines (i.e., Wild Irish Rose, Night Train, Italian Swiss, Gallo, Taylor Port

and White Port, MD 20/20, Cisco, Cook Breeze, Thunderbird, Sunset Grain Alcohol, Seagram's
Spritzer and other such fortified wine brands) or Special Brews (i.e., All High-gravity malt liquors, St
lves, Steel Reserve 211, Colt 45, Sparks, Juose, Schlitz, Axe Head, Camo Black, Cobra, Ko~loko). Also
the business will not sell wine less than $7.75, to be adjusted bi-annually based upon the November
2014 Consumer Price Index- Urban Wage Earners and Clerical Workers (Chicago All Items)
published by United States Bureau of Labor Statistics, in their original packaging from the
distributor or sell wine in a container(s) from their distributor in a quantity of less than 748mL per
unit.
5. Spirit Sales: This business will not sell spirits in a container from their distributor in a quantity less
than 375mL per unit or spirits less than $12.50, to be adjusted bi-annually based upon the
November 2014 Consumer Price Index- Urban Wage Earners and Clerical Workers (Chicago All
Items) published by United States Bureau of Labor Statistics, in their original packaging from the
distributor
6. Business Name: The name of the establishment shall be Michael's Wine & Sprits
7. Prohibited Store Items: The business will not have any Lottery Sales or other gaming activities at or
on the premises. In addition the business will not sell convenience store items (i.e., chips, candy,
etc) at or on the premises.
8. Change of Officers: The business will not change the owners and/or officers of the corporation for a
period of not less than 10 years from the date of the issuance of the license.
9. Security Cameras: Outdoor Cameras shall be installed outdoor cameras for the safety of patrons
and community members. Such cameras shall be able to record discernable images from a
minimum distance of 15 feet in front of and on the sides of any business entrance with
maintenance of such cameras and tapes or videos for a minimum of 72 hours and make such
recordings immediately available upon request of any City of Chicago agency. The system shall be
linked to the City of Chicago in conjunction with the Private Camera program administered by the
Office of Emergency Management and Communications
The conditions of the Packaged Good Liquor License issued pursuant to this Plan of Operation are legally
binding and may be enforced by the City of Chicago enforcement authorities. All other conditions of the
license are governed by the City of Chicago Municipal Code. Violation of the above stated conditions may
result in the imposition of a fine and/or suspension or revocation of all business licenses issued to the
Licensee. Violation of the above stated conditions may also result in the issuance of cease and desist orders
prohibiting the activity which violates the conditions of the Packaged Goods Liquor License.

The conditions of Packaged Goods Liquor License issued pursuant to this Plan of Operation shall apply to
the business address and Licensee and to all officers, managers, partners, and direct or indirect owners of
the licensed entity. The sale of the business to other persons purchasing the stock or membership units of
the licensed entity does not void the conditions of the license. Any and all potential new owners of the
licensed entity shall be subject to the same conditions set forth in this statement.
It shall be the duty of every person conducting, engaging in, maintaining, operating, carry on or managing
the above mentioned business entity to post this Plan of Operation next to the Packaged Goods Liquor
License in a conspicuous place at the business address.
