Hover over
"Working with
Alston", then click
on
"Subcontractors".
This will take you
to the page below:
Click on the
"Prequalification
Application" link
and it will take you
to the page below:
If this is your first time here, please register by entering the
information in the blanks to create an account and then click
the "Create Account" button, which will take you to the page
below. If you have registered previously and are returning, you
may simply log in using the email and password you registered
with.
Fill in all
information
Click on each of
the tabs to open
that section of the
form and complete
all information.
The Financial
Information,
Insurance and
Safety tabs will
require you to
upload documents
in addition to
completing the
forms.
If you must exit
before completing
all information and
document uploads,
you can save your
progress by
clicking the "Save"
button at any time
and then log out.
Upload/attach your
most recent 3
years audited
financial
statements. For
confidentiality,
these documents
are only viewable
by our CFO.
Complete all
information
Please upload/
attach your sample
insurance
certificate and all
endorsements.
Please download
and review the
standard
subcontract
agreement by
clicking on one of
these 2 links. These
are the terms of the
contract you will be
asked to agree to
when working with
us on a project.
This also lists the
insurance
endorsements you'll
need to upload with
your sample
insurance certificate
Please list your 3
most recent years
EMR and upload/
attach a letter from
your insurance
carrier confirming
this information.
Please upload/
attach your 3 most
recent years OSHA
300 and 300A logs.
Don't forget to block
out employee
names in the logs to
assure
confidentiality.
When all data has
been entered and
uploaded, Check
the "Checking here
signifies..." check
box and click the
submit button,
which will let us
know the
submission is
ready for review.
Clicking this button
before all
information is
complete and
uploaded will still
save and submit
the information but
will result in delays
to prequalification.
