“supporting happier, healthier lives” www.chainreaction-newcastle.co.uk Chain Reaction is a new service commissioned by Newcastle City Council that supports older people (55+) to live independently in the community. By promoting the use of community-based resources and facilities, Chain Reaction encourages people to lead happier, healthier lives. It supports and sustains friendships in order to combat social isolation and to increase a sense of good emotional health and wellbeing. Chain Reaction is a new approach that offers a real alternative to building-based day services; the service is flexible and personalised, and works with older people to support them in deciding what they would like to achieve and how to make this happen. The service offers a variety of support, from sorting out financial worries to organising outings. What is on offer? Any plans agreed will depend on the person and their circumstances, but the idea behind the service is to make sure that older people can get the support they need to live at home. This includes feeling safe and well, being treated with dignity and respect, being in control of making decisions, and being able to stay in touch with family, friends, and the community. The Chain Reaction service provides different levels of support, which can be flexible and change over time. Chain Reaction is provided by three different organisations: Search, KeyRing, and Mental Health Concern. Together they have specialist knowledge and experience of working with older people, those who have a learning disability, and people with poor mental health. Support can include: Prevention Life Skills Funded by Public Health £59 per week Chain Reaction Pioneers can offer low-level support with: • Accessing social activities, volunteering, training and employment • F ace-to-face information and signposting •P ositive health and lifestyle choices •D eveloping peer support and avoiding isolation People who need more support can add the following elements to the Foundation level: • domestic skills (for example, meal preparation) • budget-planning • health and wellbeing (for example, help with medical appointments, medication reminders, and getting involved in relevant support groups) Foundation £39 per week •A dvice, advocacy and liaison to get support that works (e.g. aids and adaptations, assistive technology, welfare rights) •H elp in gaining access to other services •A ccess to local community organisations and resources •R isk assessment to support people to stay safe •D eveloping peer and volunteer support networks to monitor and promote emotional and physical health and wellbeing •E motional support Refer someone to us By email: [email protected] By phone: 0191 273 7443 0300 300 7070 07917 686 658 Please call any one of these numbers and you will be referred to the organisation best suited to your needs. www.chainreaction-newcastle.co.uk
