12 FIRST AID SCENARIO Scenario Name Kitchen Burn In the galley, you and some friends are preparing the pie and peas for the post racing social. The mushy peas are boiling and are ready to be served. You pick up the pot of bubbling peas and start in the direction of the serving area. The pot holder slips slightly, causing the peas to splash on to your arm. The scalding peas burns your forearm. You are experiencing a lot of pain, with a red, blistering burn on your arm. What is the appropriate first response? This is a minor burn on the arm only, if covered more of the body, groin area, face then treat a serious burn and get medical help Place the burned area into cool water right away. Cover the burn with a clean bandage. Do not pop blisters. The intact skin protects the burned part from infection. Blisters heal on their own in a few days. The victim may take a pain reliever to reduce discomfort. Pain relieving preparations can be applied to reduce discomfort while healing. Scenario Name Try Sail You are taking someone on a Try Sail. You hear the participant say to a friend that he had never been sailing and that it was good to get out and get some exercise for a change. After sailing and you return to shore he says his left arm and shoulder are a bit sore. Suddenly he grabs his chest. He stubbornly says he wants to go out for another sail as soon as the pain passes. You go over to offer help. When you ask him to lie down in the shade he refuses because he says he feels nauseous. What are your next actions as first responder? Possible heart attack or angina. Get history and check for medication, if no history immediately send for an ambulance. Sit or lay the victim down, propping up the head. Help administer heart medication unless person is unconscious. Treat for shock. o Treat possible cause of shock – severe bleeding (could be glass in it) o Lay casualty down, preferably on a blanket away from danger o Raise feet above heart level o Get medical help o Loosen tight clothing o Keep them warm In case of cardiac arrest, perform CPR if you have been trained. 12 FIRST AID SCENARIO Scenario Name Boat tie down At the sailing club you trip on a boat tie down and you feel your ankle fold over. A burst of pain shoots through your lower left leg. The ankle joint swells and turns purple in minutes. What should the first-aid response be? Appears to be a sprain Apply R.I.C.E. Rest Ice Compression Elevation Do not use the injured part. Use ice packs to prevent swelling. Wrap the injured part in an elastic bandage Elevate the injured part above heart level. Have the injury checked by a medical professional. Scenario Name Jacquie’s cooking At the laying up supper cock-au-van is on the menu. During the meal there is lots of conversation and laughter. Josh starts to gag and looks panicked. He holds his throat and is not making any noise or coughing. What is the appropriate response? Ask the person if he is choking and needs help. If the person can talk, he is getting air. If the person can cough, encourage him to continue in order to dislodge the obstruction. Send someone for help. Get medical help. If the victim is unable to talk or barely able to cough, perform abdominal thrusts 12 FIRST AID SCENARIO Scenario Name Starboard and port rules You are teaching racing rules and on the white board showing Starboard and Port rules. The session has been going for about 15 minutes. One student suddenly becomes rigid and starts to jerk violently. The student slumps to the floor and the convulsing continues. You’re next to a shelf stacked with books and other supplies. What is the appropriate response? The student appears to be having a seizure Remove any objects from the immediate area which the person might bump. Loosen tight clothing. Call for medical help. Do not put anything into the person’s mouth while he or she is convulsing. If the person is unconscious turn the head to the side and observe if he/she is breathing. Wait for convulsions to stop. People who are unconscious after convulsions should be laid on their sides. If the person is not breathing, start rescue breathing if you are trained. Scenario Name Trip One of the instructors running the sailing course has tripped and fallen down the steps from the Race Office. He has bumped into a cupboard. He fell heavily onto the right side of their chest and cut their right forearm on one of the glass display shelves. There is a lot of blood coming from the cut on their arm and they feel a sharp pain each time he breathes. They are pale, shaking and are starting to feel quite dizzy. A direct force to the chest from a blow, fall, or crush, could fracture one or more ribs which could pierce the lung, or becomes dislodged causing breathing problems and pain whilst breathing. Fractures of lower ribs can cause internal bleeding. Ask casualty to sit down and raise arm on injured side and support arm Arrange medical assistance Do not allow them to eat or drink If they are pale, shaking and starting to feel dizzy then possible sign of shock starting to set in. As shock sets in it reduces oxygen supply to the brain. Treat possible cause of shock – severe bleeding (could be glass in it) Lay casualty down, preferably on a blanket away from danger Raise feet above heart level Get medical help Loosen tight clothing Keep them warm 12 FIRST AID SCENARIO Scenario Name Parked Car A person was walking around a corner in the car park has stepped between two parked cars The driver of the one car could not stop before the person had been knocked over. The casualty is complaining of severe pain in their upper right leg and it appears swollen and deformed half way between the knee and hip. You can also see blood coming from a cut on their upper arm. They are pale, shaking and feeling dizzy. If they are pale, shaking and starting to feel dizzy then possible sign of shock starting to set in. As shock sets in it reduces oxygen supply to the brain. If there are no obvious signs of blood yet the casualty shows signs of shock it is an indication of internal bleeding. Treat possible cause of shock – severe bleeding Lay casualty down, preferably on a blanket away from danger Get medical help Raise feet above heart level Loosen tight clothing Need to treat leg which could be fractured / broken and which you will need to immobilise. Scenario Name Buzzing The Bosun was working outside the club house and has been stung on the back of the left hand by a bee. The back of their left hand is red and painful. You notice that his eyes are red and he complains they are itchy and they are watering, his face is pale and around his eyes he starts to look puffy. It would appear that he is suffering a severe allergic reaction (anaphylactic shock) and you need to act quickly as he could deteriorate and his reaction could become fatal. Get medical help immediately If he is carrying an auto-injector of adrenaline help him take it o inject into thigh for 10 seconds o remove o massage the injection site for 10 seconds Help him sit up in a position that helps with breathing If he becomes pale and weak lie him down and raise his legs and treat for shock
