Office on Aging The mission of ERFSC is to empower families, youth, and communities to become more self-sufficient through integrated and collaborative community based services. If you would like to open a nutrition site, contact our Administrative Office and speak with our Project Director. 3917 Minnesota Avenue, NE Washington, DC 20019 Tel: 202-534-4880 Fax: 202-388-7691 www.erfsc.org This program is operated by: PROGRAM SERVICES Congregate Meals: Each day a wellbalanced meal which ensures a minimum of one-third of the daily recommended dietary allowances is given to seniors attending one of our Nutrition Centers. Additionally seniors can participate in nutrition education classes, counseling, screening and other supportive services which are provided to foster a positive effect on the life span. Case Assessment and Case Management Office: This component of the program provides case management and follow-up services for seniors. Utilizing these services enables individuals to maintain independent living standards. Counseling Services: This component connects individuals with the appropriate service(s) needed to strengthen their emotional and personal capacity for functioning which allows them to maintain maximum independence. Follow-up evaluation is provided to ensure that quality services have been delivered. Home Delivered Meals: Three days a week, a meal is personally delivered to seniors who are at home and unable to prepare food for themselves. Health Promotion: Daily seniors participate in Body wellness activities, exercise and fitness programs which are conducted at the Nutrition Centers. Additionally seniors receive proper information and guidance on how to maintain healthy lifestyles. Recreation/Socialization: Leisure time opportunities designed to enhance the quality of life, stimulate the senses, learn new skills and eliminate loneliness and isolation. Calendars of activities are available at each center. Intergenerational Program: Participants have the opportunity to participate in outreach programs with elementary school children working in a cooperative relationship to facilitate learning, sharing fun activities and exchanges of wisdom. Caregiver/Respite Support Program: This program provides family support through information, assistance and relief to those persons who are caring for loved ones who have moderate symptoms of Alzheimer’s disease and/or dementia. The Call “N” Ride Taxicab Program: This program is a coupon system, based on a sliding fee scale coordinated through Seabury Connector. Coupons can be used for any transportation purpose within the District of Columbia. Transportation Services: This service is available to facilitate access to various program activities, Nutrition Centers, Senior Center, leisure time activities and events, essential business trips, medical appointments, shopping trips and other appointments. All of the above services are coordinated through Seabury Connector. Volunteer Opportunities: ERFSC, Inc. KEEN Seniors Program offers a variety of volunteer opportunities for seniors and youth. Additionally, individuals with special skills are offered opportunities to work with our programs on special initiative projects.
