Sub-Committee Updates
A. County Health Issues
 Continuing to look at re-admissions
 Tobacco related programming – have a representative from the Tobacco Free Network
 Looking at holding a Community Health Fair on 11/19/2015
 Healthy Cooking Connections – two classes, one finishing up. Starting a new class in March
 Step Up to Stop Falls – consultant hired by HFWCNY came to the last meeting and did an
evaluation of the whole project
 Oswego Hospital is sending out a press release for the three Chronic Disease Self-Management
classes. First class will be at Bishop Commons, two more scheduled for Hannibal and Central
Square. Looking to have one up north at some point.
B. Funding & Public Policy
 Grant writing survey - defined three groups - housing, chronic disease, substance abuse
 Reached out to organizations that were interested
 Working on forming sub-groups - open to others if interested
 Advocacy Survey Results
o 19 organizations responded
o What is the best way to communicate with our elected official’s offices?
o Library of resources was started and will work on this again
o Looking at developing a RHN newsletter that will highlight a couple of RHN member
organizations in each newsletter
C. Health Workforce



Planning for the new Provider Reception
o Wednesday, May 13th from 6-8 p.m. at The American Foundry
o Save the dates and provider nomination forms going out by the end of this month
o Similar setup for food, etc. as last year
First meeting Friday for the Rural Immersion Workgroup
o Looking to develop a program similar to CNYAHEC
o Focus on getting medical students between their 1st and 2nd year of school
o Week long program around the time of the triathlon in Oswego
o Get them immersed into the community and showcase our county to entice them to come
here to work after school.
Employment Website - collaboration between TECC and Health Workforce
o Able to post positions on the site for all organizations
o Is there a resource that’s already available that we can utilize - Indeed, recruiting website,
etc.
o CNYAHEC is in the process of developing a workforce website. They will have the
second version of the website out in February.
o Looking at search engine optimization
D. Technology & Emerging Care Concerns
 HealtheConnections attending the meetings
 Meeting time change to 8:30 a.m. at Oswego Hospital in February
 Incentives available for providers
 Big push in 2015 to get providers to start sending their data

DSRIP Updates – Committee Formation






VI.
HIT Mash camp - looking at for 2015 and putting together a planning committee. How do we get
people from our county to attend?
This is a standing item on County Health Issues agenda as an informational item
 Based upon feedback at the January meeting, the committee does not feel that they
should have a DSRIP committee under them. They feel that it makes more sense to have
it as its own committee.
OCO & OH - collaborating on an oversight committee - Diane Cooper-Currier chairing this
committee. Working on a list of the 11 goals and what organizations will be providing. Is it
possible to make this committee a committee under the RHN.
 Brian will be attending the next meeting to share about the RHN Strategic Planning
process – determine if RHN should play a role in facilitating those meetings.
Start with a core group then reach out to all providers, develop a message and filter it down to
others.
Information on website with contact people and what goals they are working on
Groups are flushing out what they’re going to work on
Keep on the agenda for this committee
HRSA
OH engaged a grant writer to work on developing a RHN grant. This would be to evaluate an
integrated delivery network. Proposal went in 1/9/15 - partnering with several agencies - OCO, OH,
Farnham, Catholic Charities. Goal is to engage the consultant to develop the integrated delivery
network and engage some of the major insurers in Oswego County.
VII.
Member Updates / New Business
Gidget Stevens
 First months report had about 150 cases due to some issues they were having with the report.
Ended up with only three cases.
 Going to Albany to get an updated timeline with Medicaid administration.
Tammy Thompson
 EI capacity, reaching out for PT and ST providers. Starting to feel more of a pinch from not
doing preschool.
 Having problems with home care waivers
Greg Osetek - Working on the managed long term care issue
Tricia Clark
 Version upgrade last Thursday, everything is going well.
 New project with OH for labs, patients no longer need a paper requisition for Oswego office.
 Thursday - turning on RHIO for labs, imaging, etc., less manual upload.
 Ellen Holst complimented them on the new website.
 Patient Portal will go live by the end of the first quarter
 Electronic website for staff is up and running - communicate with them here.
Patricia Bellinger – reviewed their end of year report – saw 202 patients in 2014

Jiancheng Huang
 Experiencing staff changes, services increased and funding decreased in 2014. Looking to work
with new County Legislator this year.
 Campaign education working on smoking prevalence in Oswego County. 30% of pregnant
women in Oswego County are smokers.
 Enhancing school-based physical education - two elementary schools in Oswego County
participating in a program this year.
Cindy Walsh
 Nutritionist position is open, closes next week.
 Training will be back up and running mid-March.
 Attending the WIC conference and educating elected officials. Meet with every office over three
days.
Sue Witmer
 Cayuga in the process of selecting nursing class for Fall 2015 for Fulton and Auburn class.
 COTA program moving forward, architects on site. First class will start the Fall of 2017. Already
getting phone calls from students interested in the program. Curriculum developed, have a
collaboration with Oswego County guidance counselors. Lab will be in the old Radio Shack classroom and apartment setting.
Jimmy Hayes – RHN members are always welcome to call. Senator Ritchie has specific people set
up in the portfolio for medical.
Nicole Kolmsee
 Local health home network meeting this Friday, 2 pm here in the Training Room.
 Sponsoring another Suicide Prevention taining - March 18-20 at Phoenix Education Center.
Open to anyone in the county.
 Health homes and transition to managed care - concern is the providers don’t exist that are
needed, having trouble meeting the need right now.
Katie Pagliaroli
 Going live with electronic ordering and documentation
 Working on admission reduction.
Jackie Iacovelli
 Community award grant for $4,000 available - deadline is 2/9/15.
 Award announcement will be early March.
 Haven’t had a lot of application activity.
Strategic Planning
 Received one proposal so far.
 Will send out all proposals to the committee to review and vote on.
 Goal is to have someone selected by the end of February and have a session in March.
 PHIP grant - kick off was on Friday, advisory group should be coming together soon.
 All the sub-committees are either reviewing the work plan now or will be next month.
