What’s New As we approach the holidays, it is a great time to reflect on the year so far, and prepare for what is ahead. It is in this spirit that we are pleased to bring you news of a new service that Autumnleaf Group is now offering and an addition to our team. ∂ Our newest service, Intensive Care Coordination (ICC), is an all-inclusive team collaborative process for creating and implementing individualized care plans for children and their families. This process includes other involved systems, helping professionals and natural supports for the family, as well as support from the assigned ICC Facilitator. The program is designed to keep youth, who are dealing with behavioral or emotional challenges, in a family or community-based setting, as well as those on the verge of transitioning from out-of-home care. Using the evidence-based High Fidelity Wraparound (HFW) model, the resulting comprehensive plan provides families more control in prioritizing and obtaining the services and supports they need as a family. Through the process, the youth and family experience added confidence and improved ability to manage their lives, leading to greater selfefficacy, health and relationships. ∂ We are delighted to introduce our new staff member, Grace Park, LGSW, LSW. She is a social worker who has previously worked in therapeutic foster care and mental health in Baltimore, Maryland. Last year 4,500 children were adopted from foster care in the United States and the goal is to find families for the 100,000 children who are living in foster care now.
