Member Goals Worksheet
General Information
For each Member position the organization is requesting, complete goal sheets that adequately represent the
service the AmeriCorps member will provide to your organization based on the service description.
An application cannot be considered for funding without a goal worksheet.
ALL partners must use the 2015- 2016 LISC AmeriCorps Goal Worksheet Templates in order to be considered
for participation.
ALL partners must submit the goal worksheets in an MSWord format.
The Member Goal Worksheet is fill-able forms and as such only the information requested can be edited.
A Member may have more than one goal. If that is the case, indicate how much of a Member’s time will be
spent on each goal.
The measurements are directly related to the outputs and outcomes that LISC and our funder, CNCS, are
seeking to achieve with the AmeriCorps resource. As such, prospective partners may not change the output or
outcome categories within worksheet.
If a member is being requested to work in two areas, for example as a financial counselor and a job placement
counselor, a site would submit these two worksheets and note how much time is anticipated to be spent by the
member on each activity (75% for financial counselor and 25% job placement referrals). Please note that one
goal worksheet is preferred; but, if needed no more than two should be selected. If a site desires to have a
member serve over more than two goal areas, the organization should have a conversation with the local
administrator.
If an organization is seeking a member to perform service that does not appear to be captured by the LISC
AmeriCorps goal worksheets, speak with the Local LISC program contact and/or the LISC AmeriCorps
Program Director. There are service activities not supported by the LISC AmeriCorps program. If a goal is
developed and the service is deemed not supportable by the National LISC Program Director, then a member
cannot be awarded to the partner to perform this service.
If a site is requesting multiple slots, do not aggregate the goals. One member slot = one set of goals. The
process is competitive and it is likely that all requests can not be fulfilled. Sites should indicate the priority of
slots in the application. If no priority is indicated in the application, the local LISC office will make the
determination as to what position will be funded if resources are limited.
Completing a Member Goal Worksheet:
1. Select the appropriate Member Goal Worksheet for the type of service provided. Indicators to select the
appropriated form include:
a. The general type of service a member will perform is noted in the header of each worksheet
b. The corresponding LISC Building Sustainable Communities area is in the header
c. The stated goal of the member’s proposed service appears in the blue shaded box at the top of
each worksheet.
d. The associated outputs and outcomes provide information
2. Indicate the full name of the partner
For example, use Massachusetts Association of Community Development Corporations not MACDC
3. Provide the Member Service Title that exactly matches the title on the Service Description template
4. Indicate the type of slot requested (1700, 900, 450, or 300) by selecting the correct slot type from the
drop down list. This should correspond to the Service Description.
5. Describe the demographics proving that a low/ moderate income neighborhood is being served by the
Member in the form box.

6. Describe the activities Member will be engaged in performing in the form box.

7. Describe how often the Member will be engaged in these activities in the form box.

8. Provide an OUTPUT number.
The form box is formatted for a number only. Any text provided will default to a “0” Be sure to make
this a realistic number given the output parameters and knowing the time to bring a member “up to
speed” and the time limit of the term (no more than 10 months).
9. Repeat “STEP 8” if more than one OUTPUT is requested

Any text or percentage amount provided will default to a “0.” Be sure that this is a realistic number
given the output number provided above. For example, the OUTCOME should not also be the exact
number as the OUTPUT; a 100% OUTCOME is desirable, but unrealistic.
11. Repeat steps 1 – 9 if a member’s service is being split over multiple goal areas. Be sure to adjust
percentages of time the Member will spend in each goal area.
