RESULTS-ORIENTED WELLNESS PLAN 2015-2016 OCCC HealthNow Wellness 1 OCCC HEALTHNOW WELLNESS PLAN FY 2016 To: All OCCC Results-Oriented Wellness Plan Participants Thank you for your participation in the Oklahoma City Community College Wellness Program. Your engagement and support has resulted in an amazing 97% participation rate. To build on the momentum, we have taken your feedback and suggestions in consideration to simplify the wellness program initiatives. We have engaged partners, BlueCross BlueShield and Catapult Health to assist us in focusing this wellness plan not only on biometric measurements but also wellness, fitness, and lifestyle change components. Our goal this year is to provide the tools and resources to enable you to reach your health goals. Please review the attached materials and feel free to contact your human resources team for additional details. 2 IMPORTANT INFORMATION: PLEASE REVIEW The OCCC results-oriented wellness plan is reasonably designed to promote health and the prevention of disease. The plan is designed to meet this standard by assisting employees that are part of the group medical plan improve and measure their health. Employees will be asked to complete HRA reports, biometric screenings, and visit their primary care physician for an annual exam for the purpose of alerting them to health risks which meet the health promotion and prevention standards. This plan is voluntary for all employees enrolled in the College health plan, and participation is designed to enable employees to earn credits or points to help mitigate the cost of their medical insurance premiums. Employees are not required to participate nor does failure to participate exclude them from full coverage under any of the College group medical plans nor will any adverse employment action or retaliation against, interference with, coercion, intimidation or threatening of employees occur to those who do not participate or who fail to achieve certain-health outcomes. Those electing to opt-out of the results-oriented wellness plan may be required to pay the employee’s portion of the medical premium for the following plan year. As a participant of the wellness plan, employees may be required to provide requested medical information to a contracted third party provider that will obtain and secure the required medical data via a secure web-based portal available to employees for submission of information or by securely obtaining the data directly from the college health coverage providers. The medical data is restricted from disclosure. All third-party partners Medical information obtained by the third-party provider will only be disclosed to the employer in aggregate form. This plan is not designed or intended to discriminate against employees with disabilities or on the basis of race, color, sex, national origin, religion, compensation, age, pregnancy, generics information or any other grounds prohibited by law. OCCC will provide reasonable accommodations that enable employees with disabilities to fully participate in wellness programs, and earn any reward or avoid any penalty offered as part of this program. If you have any questions or concerns, please contact Dr. Angie Christopher, Vice President for Human Resources at (405) 682-7890. 3 OCCC HealthNow Wellness Premium Waiver Criteria OCCC employees participating in the College Health Plan will have from July 1, 2015 to May 1, 2016 to complete the premium waiver criteria. Participation in the Results-oriented premium waiver system is voluntary for all OCCC employees participating in the College Health Plan to earn health premium waivers. Employees participating in the health plan that elect not to participate with criteria points will be responsible for $200 of the employee monthly health premiums effective July 1, 2016. Participating employees must accumulate 8 criteria points no later than May 1, 2016 to earn 100% waiver of the employee’s monthly health premium cost. Employees that complete criteria points will earn $25.00 credit for each criteria point earned up to $200. All new employees hired after January 1, 2016 will be required to accumulate 3 criteria points no later than May 1, 2016. That is why we have partnered with Catapult Health to provide free, quick, and confidential preventative health checkups right here at the College during the month of October. These checkups will be available to all employees covered under the OCCC health plan. If you think you are healthy, fantastic! This 30 minute checkup will help confirm that. But if you have any warning signs or risk factors, this checkup can help you identify what is going on. A licensed Catapult Health Nurse Practitioner will help you create a personal action plan for improving you health, if you like. Catapult Preventative Checkups focus on the most common and costly chronic diseases. Each checkup includes lab-accurate diagnostic blood work, a fully tailored Personal Health Report provided at the time of the checkup and a private session in a closed door room with a licensed Catapult Nurse Practitioner. Your individual results are strictly confidential and no one at OCCC will have access to your personal health information. We want all of our employees to be healthier, happier, and more productive-at home and at work. Private and confidential comprehensive and tailored personal health report for participant. Private and confidential health report results reviewed with an onsite Nurse Practioner, who will review any risk factors identified. Voluntary, Flu vaccinations 9 SAVE THE DATES IMPORTANT: The 1st week of August, Catapult will make the Primary Care Provider Forms (PCP) available for you to take to your doctor to complete during your annual physical exam. You will send the completed PCP form to Calapult through their secure fax line for processing. Catapult will follow-up with each participant within 48 hours to verify delivery. What if I already had my physical for FY 2016? No problem. Just hold on to the form that your doctor completed and send it through the secure fax line set up by Catapult for our group. 10 Next Steps?
