Mr. Webb:
On June 9, 2005, an inspection was performed on the above referenced SWD System in accordance with
Department policy. It is determined that the system is not in compliance with the terms and conditions
set forth in the above referenced permit.
Specifically, the following was found:
(1)
No identification of system or facilities. System should be identified with sign indicating
Operator, System Name, Location and Permit Number. All facilities should be properly labeled
with Operator and Name.
(2)
Firewall drains were not plugged. Drain valves should be closed and plugged while unattended.
Buildup of salt was observed inside S.W. firewall with evidence of release of salt water from
firewall causing vegetation kill. Only fresh water can be discharged to ground. Minor vegetation.
(3)
A fairly large oil spill was observed at the Parker No. 2 SWD Well. The spill is migrating off site
into woods. Needs remediation.
This office must be notified, in writing no later than July 8, 2005, with an outline of your corrective
actions to the above non-compliance items.
In addition, please be advised that this SWD System must be constructed, operated, and maintained as
permitted. Routine maintenance and remediation must be performed as needed. Annual permit fees
must also be current to maintain compliance.
