Healthwatch Oxfordshire Hospital Discharge Project
Healthwatch Oxfordshire are trying to find out about local people’s experience of
being discharged from hospital.
What we want to know:
•
What is and isn’t working well about hospital discharge, from the patient
and carers point of view?
•
How does the way a patient is discharged help or hinder the organisations
trying to support people after they have been discharged?
We will find this out by:
•
Using our rights to Enter and View in order to ask patients, clients and
carers about their experience during the discharge process. And conducting
these visits in the setting where their care is being provided.
•
Carrying out some focus groups with people who might prefer that to filling
in a survey
•
Asking the public to get online and to fill in an online version of the same
survey
•
Asking the care workforce what they think through their own online survey
•
Analysing what we are told
Then we will:
•
Make some clear recommendations to providers and commissioners about
where there is great practice to build on, and where improvements are
needed.

How you can help:

Tell Healthwatch about your experiences by filling in the feedback form on
the Healthwatch website now. This will help inform the questionnaire
design.
•
Apply to be a Healthwatch Oxfordshire enter and view volunteer, so that
you can go out and ask people about their experiences for us.
•
Suggest sites for enter and view visits.
•
Promote the project and encourage people to fill in online questionnaire
when it’s published.
