1. Vehicular Accidents in Tanauan, Batangas

On or about 4:46 PM, 21 November 2012, a vehicular accident occurred at KM 64+000
STAR Tollway, Brgy. Trapiche, Tanauan City, Batangas involving an lsuzu Elf with plate
number TJW-891 , driven by Ronald Marasigan y De Guzman, 34 years old, a resident of
Labate, San Juan, Batangas and a trailer truck (Gas Tanker) with plate number RKK-226
driven by Joel Melgar y Ganes, 41 years old, resident of Cut-Cut First, Capaz, Tarlac.
Based on the investigation, both vehicles were travelling northbound when said trailer
truck accidentally bumped the rear-end portion of lsuzu elf. Driver of trailer truck sa id to
have fallen asleep which caused the incident.
As a result, both drivers and one occupant: Luzviminda Macaraig y Castor, 54 years old,
resident of Brgy. Libato, San Juan, Batangas sustained minor injuries and were brought to
Cabrini Medical Center for medical treatment.

OCD-RDRRMC IV-A coordinated with PDRRMO Batangas, MDRRMO Tanauan, STAR
Tollway and PNP Tanauan regarding the incident.
Star Tollway, Tanauan City Police Station and three (3) fire trucks from Sto. Tomas and
Tanauan City responded to the incident.
2. Flooding Incident in the Municipality of Katipunan, Zamboanga del Norte

Last November 10, 2012, the Municipality of Katipunan experienced Strong winds and
flashfloods to the area believed to be caused by tornado. A total of 17 barangay namely:
Barangay Matam, Maitam, Dabiak, Sanaa, Morayao, Daanglunsod, Seroan, San Vicente,
Carupay, Biniray, Seres, San Antonio, Singatong, Dos, Sitog, Dr. Jose Rizal and Sto. Nino
where affected and several private properties, agriculture and infrastructures were
damaged.
There were no reported casualties .
.Affected families temporarily stayed on their relatives/friends houses but now back to their
respective houses.

The Municipality of Katipunan declared a STATE OF CALAMITY with resolution number
12-200 s., 2012.
Katipunan MPS together with other volunteer groups/organizations are helping in the
conduct of search and rescue to the victims.
The MDRRMC of Katipunan provided assistance such as food assistance and shelter to
the victims together with the help of the LGU and volunteer organizations.
MDRRMC of the affected municipality is continues to assist the victims of the f lood
together with the help of LGUs and private organizations.
OCD-RC 9 Operations Center continuously monitoring the situation and constant
coordination with the affected MDRRMC for any development.
3. Vehicular Accident in lbaan, Batangas

On or about 1:00AM, 21 November 2012, a vehicular accident occurred at KM 96+100
STAR Tollway, Brgy. Quilo, lbaan, Batangas involving Mindoro type jeep with plate
number VAE-893, driven by Leonardo Paglinawan y Cabra l, 46, resident of Brgy.
Personas, Calapan City, Mindoro and a Hino 14-Wheeler Truck with plate number RGK671 , driven by Jimmy M. Borjal Jr., 27, resident of Brgy. San Roque, San Isidro, Nueva
Ecija.
Based on the investigation, said truck was travelling southbound when it overtook a
vehicle ahead which caused head to head collision with the jeep which was travelling
northbound.
As a result, one (1 ) died while seven (7) were injured and were brought to Batangas
Regional Hospital, Batangas City for medical treatment:

lbaan regarding the incident.
Star Tollway and lbaan Police Station with Provincial Highway Patrol Team (PHPT)
responded to the incident.
4 . Progress Report re Reported Collapsed Mining Pit at Paracale, Camarines Norte
Situation Overview
• On or about 9:00 PM, 20 November 2012, an alleged dynamite explosion that took place
long the mining area which was said to have originated from the guerilla-type mining
activities at Sitio Bulaay, Purok Maligaya, Barangay Palanas, Paracale, Cama rines Norte.
• The incident allegedly resulted to the collapse of one (1 ) mining pit/tunnel.
• The supposed financer of the alleged mining activity, Agosto Jordan of Barangay Palanas,
Paracale, Camarines Norte, refused to cooperate or give any information to confirm I deny
the incident.
• Progress report as of 4:30 PM, 21 November 2012 indicates that the mine collapsed as a
result of sudden influx of water brought about by high tide.
• No casualty has been confirmed as of this writing .

Dispatched OCD 5 personnel and one (1) team BFP-SRU with SAR equipment to
assist in the conduct of Search and Retrieval operation as requested by LGU
Paracale, Camarines Norte.
LGU Paracale thru the Local Disaster Risk Reduction and Management Office have
organized local volunteers in a search and rescue operations since 20 November
2012 but no trapped miners have been rescued or retrieved . They have requested
assistance from the OCD 5 in the conduct of SAR operations.
OCD 5 has coordinated with BFP-SRU to conduct ocular inspection on the mine
collapse site and assess the situation for whatever assistance might be appropriate
and is continuously monitoring the situation.
