C A S E S T U D Y BIOFIBER™ Strips/Discs: Absorbable Biologic Surgical Mesh Case Overview Patient is a 64 year old male with a one year history of shoulder pain. Prior to surgery the patient had significant pain, elevation to 130 degrees, external rotation to 30 degrees and marked weakness. He has tried medication, injection, and physical therapy with continued pain. His MRI demonstrates a full thickness medium size tear of the supraspinatus extending into the infraspinatus tendon with retraction. (Figure 1) John W. Sperling, M.D. Professor of Orthopedic Surgery Mayo Clinic Rochester, MN Figure 1 Surgical Technique KEY POINTS: Biomechanical strength No concerns about human or animal tissue Ease of storage and use At the time of surgery, the rotator cuff was noted to be thin in nature with significant delamination. Due to the quality of the rotator cuff tissue, the decision was made to perform a double row repair with BIOFIBER™ surgical mesh augmentation. The patient had double loaded 2.8mm PITON™ anchors placed in the medial row. (Figure 2) Horizontal mattress sutures were placed through the rotator cuff and the BIOFIBER™ surgical mesh and tied. (Figure 3 and 4) These sutures were then tensioned laterally using 3.5mm PITON™ anchors. (Figure 5) The BIOFIBER™ surgical mesh resulted in a more secure repair, minimal increase in operative time, and allowed for a more rapid therapy program. Competitively priced Figure 2 Figure 3 Figure 4 Figure 5 Patient Follow-Up At four months, the patient was very satisfied with the procedure. He had marked improvement in his pain. On examination, he had 150 degrees of active elevation, external rotation to 40 degrees, and excellent strength. Technique Benefits 2.8mm PITON™ »» Small, minimally invasive insertion – allows greater bone area for tissue healing »» Strength of a 5.5mm anchor in only 2.8mm »» No instrumentation needed – no tapping, punching, or pre-drilling »» Pre-loaded with #2 FORCE FIBER™ sutures 3.5mm PITON™ »» Ability to independently cinch and re-tension the repair »» Excellent pull-out strength »» No instrumentation needed – no tapping, punching, or pre-drilling BIOFIBER™ Surgical Mesh »» More secure repair with excellent suture retention and handling »» No concerns about human or animal tissue »» Ease of storage and use »» Priced competitively »» Can be used to reinforce the repair on the underside or top of the tendon »» Made from biologically derived fibers PITON™ Knotless Fixation Implant BIOFIBER™ High Strength Suture 1023 Cherry Road Memphis, TN 38117 800 238 7117 901 867 9971 www.wright.com 10801 Nesbitt Avenue South Bloomington, MN 55437 888 867 6437 952 426 7600 www.tornier.com ™ and ® denote Trademarks and Registered Trademarks of Wright Medical Group N.V. or its affiliates. ©2016 Wright Medical Group N.V. or its affiliates.
