CBR 2106 Survey - $50 Visa Gift Card Drawing Rules
Four winners will be randomly selected from all entrees received as of the date of the drawing for a $50 Visa
gift card.
Med Center Health (MCH) employees are not eligible to win the Visa gift card. If an employee is drawn as one
of the winners, Marketing will send the employee a Med Center Health promotional item and draw again until
four non-MCH employees are selected.
The drawing will be held December 2, 2016.
Each winner will be contacted using the contact information provided on their entry (last question of the CBR
2016 online survey). The prize will be awarded to another participant if the winner cannot be reached within
14 days of the drawing.
One Visa gift card in the amount of fifty dollars ($50) will be awarded to each of the four winning participants
and will be mailed to the participant within two weeks of the drawing. No transfers are allowed.
Prize drawing is open to U.S. citizens and Kentucky residents 18 years and older.
Prizes may be subject to tax. Please speak to your financial advisor to determine if tax is owed.
Employee’s family members are eligible to win the prize, but the survey must be completed by the family
member and not a Med Center Health employee.
