Calgary Rural Primary Care Network Primer
Who is the Calgary Rural PCN?
What can the Calgary Rural PCN do for you?
• Collaboration with member physicians and
Alberta Health Services (AHS) throughout 12
core communities and surrounding south /
eastern areas around Calgary.
Primary Care and Chronic Disease

• Established in 2006, the Calgary Rural PCN
will be celebrating their 10 year anniversary in
2016.
• 141,000 people live in the catchment area,
with approximately 106,000 of them being
patients (approximately 75%).
About our Physician Members
• Calgary Rural PCN started with 59
Physicians, there are currently 135 members,
14% which are accepting new patients.
PCN Funding
• The PCN receives almost 100% of their
funding from Alberta Health – all services
are free of charge.
• PCN staff work with physician members to
prevent, manage and educate patients about
chronic diseases (e.g. hypertension, obesity,
pre-diabetes etc.), enhance continuity of care,
improve access and provide earlier
intervention.
• As part of a patients’ Medical Home a
physician may refer a patient to a member of
our health team such as: Licensed Practical
Nurse, Registered Nurse or Registered
Dietitian to help them achieve their maximum
health.

• Initiatives aimed at promoting health,
preventing disease and injury while
addressing gaps in health services.
• In addition to the care a patient received from
their doctor, the following programs and

Throughout this four week program, you work
with a group of other people, you are taught
strategies for staying off tobacco by a
Registered Respiratory Technologist (RRT)
Programs offer:

Weight Control
Making the decision to quit smoking is the first step.
Quitting is a process that required making a plan
and sticking to it.

The Craving Change™ program is not a diet. It does
not teach you what, when, where, or how much to eat.
It focuses on WHY you eat the way you do.
As you become more aware of your own personal
eating triggers, you will be better able to control
your food cravings and change your problematic
eating behaviours.
The cognitive-behavioural approach is used in Craving
Change™. This approach is widely supported by
years of research as helpful for managing problematic
eating. The cognitive-behavioural model encourages
you to consider how your thoughts, emotions and
behaviours all interact.

About Pedometers
Pedometers are available at the Bank Athletic Club
(137 2 Ave, Strathmore)
Measures steps, aerobic steps and minutes,
distance in miles and calories burned
Increase your steps by 500 each day as tolerated
(attempt to reach 10,000 every day)
Most adults who take 10,000 steps each day will
burn 2,000 - 3,500 calories in a week

Walk for the health of it is a primary prevention
