About our Physician Members • Calgary Rural PCN started with 59 Physicians, there are currently 135 members, 14% which are accepting new patients. PCN Funding • The PCN receives almost 100% of their funding from Alberta Health – all services are free of charge. • PCN staff work with physician members to prevent, manage and educate patients about chronic diseases (e.g. hypertension, obesity, pre-diabetes etc.), enhance continuity of care, improve access and provide earlier intervention. • As part of a patients’ Medical Home a physician may refer a patient to a member of our health team such as: Licensed Practical Nurse, Registered Nurse or Registered Dietitian to help them achieve their maximum health.
