DISTRIBUTION OF MATERIAL FOR PARTICIPANT GOODIE BAGS
All “8th Annual RACE 2 CURE PH –Taylor Caffrey Memorial 5K Run/Walk” participants will
receive a “goodie bag” at the event. If you would like to have product samples or coupons included
in the participant goodie bag, please complete this form. There is no fee for distribution of above
mentioned items in our goodie bags, but donations are appreciated. If making a cash donation,
please make checks payable to the “Pulmonary Hypertension Association”. If making an “in
kind” donation to our Silent Auction or Opportunity Drawing, please complete the Donation
Verification and Valuation form. Please send or drop off the completed forms, donation, and
goodie bag materials to:
