Dog Training/Boarding Contract and Release of Liability
I,
am the owner/authorized agent of the above named dog and find all of Dan
Perata’s facilities safe and adequate to keep said dog for the purposes of field training and/or boarding.
Owner/agent authorizes Dan Perata to the use of veterinary hospital of their choice to provide all veterinary and/or
emergency care necessary for the health and well being of said dog during the training period/boarding period.
Owners/agent agrees to pay any and all expenses related to that care, including any special needs listed below.
Dog Training: Owner/ agent agrees to pay training fees as follows. Training will be billed at a fee of:

agrees if said dog needs additional training after the above-specified period, it will be billed at the same rate for an equal
amount of time. The first hour/day/week/months deposit for training fee of:

time this agreement is signed. Invoice deposits are due at time dog begins training/boarding and payment in full is due at
completion. Owner/agent understands that Dan Perata, its owner(s), employee(s), or agent(s) make no warranties or
guarantees, express or implied, as to the performance of said dog following the training period.
Dog Boarding: Owner/agent agrees to pay boarding fees as follows. Boarding day rate is:

Owner/agent agrees to pay all training fees and incurred expenses, boarding fees and incurred expenses,
veterinary fees and any other expenses in full before said dog will be released to go home.
If any invoice is more than 30 days late, and owner/agent cannot be contacted, Dan Perata will consider said dog to be
abandoned. A certified letter will be sent demanding payment within 10 days from the date of receipt of the letter. If
owner/agent does not pay the past due amount in full within the 10-day period, said dog will be taken to an animal shelter.
Owner/agent agrees to provide any special care supplies (medications, heartworm prevention, flea and tick prevention,
special diet, supplements, etc.) at their expense and authorize the administration of said special care supplies.
Owner/agent agrees to pay for any ancillary products and services requested. Owner/agent agrees to provide and/or pay
for food for said dog during the duration of boarding/training.
Owner/agent accepts responsibilities for any damages the above named dog may cause through malicious, aggressive or
improper behavior and will reimburse Dan Perata in full for the value of those damages.
Owner/agent agrees to release Dan Perata and hold harmless its owner(s), employee(s), or agent(s) from any
responsibility and/or liability for injuries, conditions, illness, escape or loss of life to the above named dog that may occur
during boarding, training, transportation or competition or because special care supplies were not provided by
owner/agent.

Customer Standard Agreements:
1.
Dan Perata Training (DPT in this agreement) agrees to exercise due diligence in the care of my dog. In
addition DPT agrees to keep its premises sanitary and properly enclosed. My dog will be cared for by DPT
training staff only, without liability on DPT’s part for loss or damage from disease, theft, fire, death, escape,
injury or harm to persons, other dogs or property by my dog, or from unavoidable causes, provided due
diligence and care have been exercised by DPT.
2.
Should my dog become ill or seem to be in need of medical consideration, DPT reserves the right to
administer aid and/or use any available veterinarian. I shall pay any expenses so incurred in addition to fees
incurred for boarding services provided by DPT.
3.
My dog is in good health, and valid proof that he/she is current on Rabies, DHLPP, and Bordatella and
Parvovirus vaccinations will be provided before she/he can stay at DPT. In addition, my dog is on a scheduled
flea and tick prevention program. Dogs arriving with fleas and/or ticks will be bathed/treated at owners’
expense.
If your dog becomes ill it may not return until it is well, or has clearance from your vet that it is not contagious.
DPT cannot risk the health of other animals in its facility.
4.
I agree that my dog may be videotaped, photographed, and/or recorded. DPT shall be the exclusive
owner of the results and all such tapings, photography and recordings will be used only by DPT in its own
promotion, advertising, sale or publicizing.
5.
DPT may ask that you fill out a survey and information sheet after training is completed.
6.
Each weekend that your dog is sent home, you will be given “homework” to work on with your pet. It is
critical to the immediate and continued success that you work with your companion during the weekend. If this
protocol is not followed, DPT cannot guarantee the successful outcome of the training program. Be prepared to
spend up to 30 minutes during Friday’s pick up time to go over the skills that need to be practiced and worked
on during the weekend.
6.
I agree that my dog will be picked up or delivered (when possible) by Friday between the hours of 4pm7pm, and will be returned on Monday between the hours of 7am-10am unless previous arrangements have
been made with email confirmation ([email protected]) to stay for the weekend. If arrangements are not
made, owner will incur fees of $250 per night, or $500 max per weekend. **Please make every effort to contact
DPT if a regular pickup or drop off time is unable to be adhered to.**
