Men of Distinction Awards
“A Father’s Heart” Conference 2017 is designed to foster conversations which
promote positive fatherhood interactions, reduce black infant mortality, offer
resources and provide health education for the overall well-being of the family
unit. The Men of Distinction Awards is a perfect opportunity to celebrate the
accomplishments of those men and fathers who impact our community. Please
consider the following category for your nomination:
DISTINCTION AWARD IN PUBLIC SERVICE
Criteria: This award is the highest form of recognition presented to an individual
who has demonstrated leadership in public service. To nominate, please provide the
following:
• Nominator’s Name and contact information
• Nominee’s Name, short bio and contact information
• Identify and briefly explain the nominee’s outstanding contributions to their
community.
• Briefly summarize the nominee’s breadth of impact and the change outcomes on
individuals, small or large groups that resulted from his advocacy and efforts for
the under-served communities.
DISTINCTION AWARD IN YOUTH-LEADERSHIP
Criteria: This award is the highest form of recognition presented to an individual
who has demonstrated leadership in youth leadership. To nominate, please provide
the following:
• Nominator’s Name and contact information
• Nominee’s Name, short bio and contact information
• Identify and briefly explain the nominee’s outstanding contributions to their
community.
• Briefly summarize the nominee’s breadth of impact and the change outcomes on
individuals, small or large groups that resulted from his advocacy and efforts for
the under-served communities.

Criteria: This award is the highest form of recognition presented to an
individual who has demonstrated leadership in education. To nominate, please

community.

Criteria: This award is the highest form of recognition presented to an individual

Please send nomination form with all of the required supporting documents for each person
that you are nominating. Make sure that you clearly identify which category the nominee is
being nominated. Deadline for Submission – May 07, 2017, the nomination forms will
be available to complete electronically online as well as paper copy.
This is a firm deadline in order for the committee to review the nominee’s forms and to
tabulate votes in a timely manner so that the finalists in each category and the person that
nominated them can be notified and then submit a headshot for the program and they can
notify family, friends and colleagues.
