Important Milestones

Important Milestones
Reached
More than $20 Million Awarded Since 1998
To our Friends and Supporters,
It is hard to believe that 15 years have passed since
Hospice Foundation opened its doors to be the premiere funder of hospice care and other
end-of-life services in Monterey and San Benito counties. While much has been achieved
and accomplished since then, change has continued to shape the landscape.
At the time Hospice Foundation was established in 1997, there was just one hospice
service provider in the area. Over the years, as the community’s needs have grown, other
nonprofit hospice service providers opened. All the while, Hospice Foundation has been at
their side, funding programs to strengthen and improve their abilities to provide care in our
area. As it was then and remains today, our mission is to ensure that care is available for
families when they need it most.
In 1998, after its first full year of fundraising, Hospice Foundation grants totaled
$509,100 to six local nonprofits that provided an array of compassionate end-of-life services.
Last year, in October 2012, Hospice Foundation awarded grants totaling $1.1 million to
11 end-of-life care nonprofits. This brings to more than $20 million the total funding
awarded locally to strengthen and improve hospice and other end-of-life care in our region.
Because of you, residents far and wide in our community now have a hospice program
nearby. With Hospice Foundation funding, palliative care services are now available in
all four regional hospitals, and terminally ill children and their families have specialized
services available locally. More grief support programs are available, addressing a range of
needs, and there is greater awareness about hospice care in the Hispanic community.
Finally, more health care and social service professionals, volunteers and members of the
general public are aware of the emerging issues in end-of-life care because of our education
programs. Your support has made all of these improvements possible, and has made a
positive impact on many, many lives.
These are signigicant achievements. Hospice Foundation could not have accomplished
these goals or reached these milestones without your strong support. And, as our
community’s needs grow, local nonprofit hospice providers will continue to turn to us for
ever-increasing help to fund the work they do each day for patients and families. As we have
for 15 years, with your support, we will remain a strong and vital partner in their efforts.
We are grateful for your support, guidance and commitment year after year, and look
forward to your continued support of our important mission.
Thank you,

FAMILY FOCUS
Crossing Paths to Destiny
“Love is our true destiny. We do not find the meaning of life by ourselves alone-we find it with one another.”
American author Thomas Merton once wrote, “Love is our
true destiny. We do not find the meaning of life by ourselves
alone-we find it with one another.”
This could be the anthem of Terry and Curtis Hilliard’s
lives. Though they met for the first time as teenagers in 1959,
distance and circumstances kept them on different soul
journeys for decades. Then one day true destiny arrived.
Their story begins in the era when teens were “cruising Main”
in their cars and hanging out at Mel’s Drive-In on South Main St.,
close to Salinas High School. Terry Evertson was just 14 when
she met 6 ft. 4 inch Curtis, then 19, and was instantly smitten.
Curtis had graduated from Salinas High the year before,
class of 1958. Terry wouldn’t start there for two more years.
“We became friends,” she said, “He was always very protective
of me.”
After one year at Hartnell College, Curtis left for Texas in
the summer of 1961 to live with his uncle. Terry and Curtis
wrote to each other occasionally.
“I finished high school and just two weeks later married my
boyfriend of two years. I worked at the A&W Drive-In until
I had two daughters, Kim and Karen. While they were growing
up, I ran Cozy Care Day Care in my home for 16 years.”
In 1968, nine years after leaving for Texas, Curtis returned
to Salinas. “He looked me up and we had a friendly visit,” said
Terry. “I was married and raising my kids which were my priority
at the time.” Curtis went on to find a job at a tire company, got
married, and raised two boys of his own, Kevin and Corey.
He later became an equipment hauler and drove a truck up and
down the Salinas Valley.
“We both lived in Prunedale and didn’t know it,” said Terry.
“Our four kids even came to know one another as schoolmates. It
was a real surprise to see Curtis at Echo Valley School when my
daughter and his son were both graduating from the eighth grade.”
Over the next 20 years, Curtis and Terry occasionally saw one
another around town, always exchanging friendly greetings.
In 1980, Terry’s 17-year marriage ended. A second marriage
lasted five years. By then, she was working for Monterey County
Office of Education in Special Education.
Meanwhile, Curtis’ 14-year marriage ended. Then one day,
in 1989, Curtis saw Terry’s bright yellow Dodge Charger parked
outside of the grocery store. He went inside to look for her but,
instead, found Terry’s daughter working there. He asked about
her mom and was delighted to learn that Terry was single.
Shortly thereafter Curtis called her and, after so many years,
their timing, distance and circumstances finally aligned.
“It just all felt so natural,” Terry recalled.
They married in February, 1990 and their lives filled with
camping trips, busy jobs and hobbies. “Family gatherings at
home and new grandbabies was what it was all about,” Terry
said. And, after several years, they even bought a second home
in MiWuk Village, north of Sonora, with the intention of one
day retiring there.
But it was not to be. Their lives took an unexpected turn in 2010.

Destiny continued from page 3
“Curtis was slowing down but attributed it to just getting
older,” Terry said. “In October that year he became jaundiced.
That began a series of visits to specialists and tests. Then we
received the devastating diagnosis of bile duct cancer.
“I’ll never forget the day: November 10, 2010. We were at
Stanford Medical Center. They told us it wasn’t curable but that
they would try to slow down the progression of it.”
Over the next 14 months, Curtis underwent several surgeries, many rounds
of chemotherapy and blood transfusions.
Throughout the process, Terry said, he
bravely endured difficult complications,
infections, and a series of other discomforting problems.
“He always remained cheerful and
polite to his doctors and nurses. They all
loved him.
“He told me that he had always wanted
Cruising in Alaska
to see a glacier, so I booked a cruise to
Alaska for us in May of 2011. We planned the
trip between chemo rounds. His oncologist was instrumental in
getting Curtis strong enough for this trip. He was tired a lot, but
he did really well. We had a wonderful time, going to the comedy
shows and on the shore excursions. He got to see his glacier.”
In October, Curtis underwent additional surgery, CT scans,
and more difficult procedures.
“He was in the hospital for three weeks,” Terry said. “It was at
the end of that stay that he decided that if they couldn’t help
him anymore he didn’t want to go through any more procedures.
“We mainly wanted to keep him comfortable. We talked
about hospice with his doctor, and his two brothers were very
supportive. They knew it was what he wanted. We felt it was
time to let it all be in God’s hands. A social worker and nurse
from the hospice program came to see us.”
Terry learned that with the help of the team from Hospice
of the Central Coast, Community Hospital’s hospice program
(which receives funding from Hospice Foundation), Curtis
could remain comfortably at home. His pain and other
symptoms would be closely monitored and controlled.
“Right away I knew it would be helpful because I was worried about getting him up the steps into our home. The nurse
assured me that she would meet me there when we came home.
Sure enough, two days later when he was released, she was
waiting for us when we arrived home.
“After that, a nurse came twice a week and explained how to
use the medications and supplies. They taught me how to care
for his surgical wounds. If I had questions they were always just
a phone call away. The aides came twice a week or more. They
showered him, changed his bandages and checked him over.
“Curtis loved his recliner and preferred to sit there day and
night. He used a walker to get around. We had a small house
so we eventually put the large hospital bed in the living room
where he could be in the center of things.
“A social worker came once a week. She was very encouraging
and understanding and would let us talk about anything we
wanted to talk about. A chaplain visited to
let us know he was always available too if
we needed him. He also talked to us about
the difficult subject of the end of life. And
he prayed with us.
“They were all so good about explaining
things, and were always so cheerful. If
Curtis needed medications or supplies, the
nurse would get on the phone and order
them that night and the next morning they
were delivered to our door. To me that was
huge—to not have to go looking for supplies, and Medicare took care of all of it.”
The hospice team helped Terry take care of Curtis at home
for 61 days until he died peacefully on January 5, 2012 with
Terry and her brother by his side.
“In the weeks afterward, I received phone calls and letters
from the hospice group, checking on me and giving me information about grief group programs,” she said.
Terry also learned about drop-in grief counseling sessions
offered by Visiting Nurse Association & Hospice in Salinas
which was closer to her home. And last November, she attended
Hospice Foundation’s Coping With Grief at the Holidays workshop in Monterey (see page 9). There, her name was randomly
drawn as the winner of a Trees of Life light (pg. 8), which she
named for Curtis. She attended the tree lighting on December 13,
in Monterey.
Recalling her experience in electing hospice care for Curtis,
Terry said, “they have so much to offer the patient and the
families, it is the sensible thing to do. It was Curtis’ choice not
to go through any more procedures. Our whole motive was to
maintain his comfort and dignity. Hospice (care) was always
our Plan B.”
Terry gains strength every day, finding comfort from visiting
her 10 grandchildren and five great-grandchildren, and in helping
others who have lost loved ones.
“After my hospice grief group meetings, I feel so much more
confident with what I’ve learned.”
She says her faith reminds her that she and Curtis will one
day cross paths again.
Funding from Hospice Foundation helps make hospice care possible for many local families on the Central Coast each year. Your donations
to Hospice Foundation become the source of grants that help sustain and strengthen these services locally so that all members of our
community experience compassion, comfort and dignity at the end of life. When medical science can no longer add days to life, hospice
care adds more life to every day.
Supporting a Network of Providers
Hospice Foundation grants continued to have a major impact on the availability of hospice care in Monterey and San Benito counties
during 2012. And, in keeping with our mission, funding also extended beyond hospice to meet the array of other community needs for
end-of-life care. This includes palliative care, an emerging end-of-life care specialty; and community outreach and education for professionals. In addition to strong hospice support, below are other highlights made possible with Hospice Foundation funding for 2012:
2012 Funding Highlights
Caring for Veterans
Children’s Care
Community Education
More than 300 U.S. military veterans
and their families learned about local
hospice services and where to find help
at We Honor Veterans, a conference
funded by Hospice Foundation and
hosted in collaboration by Visiting
Nurse Association & Hospice, Inc.
and Hospice of Santa Cruz County,
at Cal State University Monterey Bay.
Hospice Foundaton will host a followup conference for professionals who
care for veterans on April 17 at CSUMB.
Critically and terminally ill children
and their families received a range of
support services through the innovative
Pediatric Collaborative, a partnership
funded by Hospice Foundation that
includes Children’s Hospice and Palliative
Care Coalition, Coastal Kids Home Care,

Support Services. Together, these agencies provide specialized care services
at home for critically ill children and
support for their families.
More than 250 local health care,
senior and social service professionals
and volunteers turned out for
Compassionate Care Alliance’s day-long

Fall Grant Awards Total $1.1 Million
In October, 2012 Hospice Foundation awarded $1.1 million to 11 local end-of-life care providers for 2013 program support. We congratulate the following organizations that received funding, all of whom are keeping pace with community needs that are ever-changing and
growing. We are proud to support the following endeavors that strengthen end-of-life care in our region:
Home Hospice Care and at Westland House
End-of-Life Nursing Care for Children
Community Hospital of the Monterey Peninsula—$441,250
Coastal Kids Home Care—$100,000
“The generous grant makes it possible for us to continue
providing end-of-life services to individuals and families in our
region. Since many Hospice of the Central Coast patients are
financially disadvantaged, we rely on Hospice Foundation to
help us continue to serve all those in need regardless of their
financial circumstance. We are very pleased with our 15-year
partnership with the Foundation and are grateful for their hard
work on behalf of our patients.”
—Al Alvarez, FAHP, Chief Development Officer
“In the continued uncertain financial times and dwindling
funding, Hospice Foundation has committed to supporting
children with life-threatening diagnoses and has taken the lead
to ensure local pediatric palliative care agencies are supported
and thriving.”
—Margy Mayfield, RN, BSN, Administrator
Home Palliative Care, Veterans Outreach
Central Coast Visiting Nurse Association & Hospice, Inc.—$250,000
“This funding is helping to sustain our palliative care
program which gives patients at the end of life and their families greater flexibility with their plan of care at home. Now they
can face end-of-life issues on their own terms. We especially
appreciate our partnership with Hospice Foundation to reach
out to local veterans.”
—Laurie Primavera, Home Health & Hospice Administrator
6
Family Support
Jacob’s Heart Children’s Cancer Support Services—$75,000
“The families of Jacob’s Heart are so grateful for the funding
provided by Hospice Foundation. Without this support, over 50
families of children with cancer would not receive much-needed
financial and emotional support.”
—Tiffany Lloyd-Lofton, Deputy Director
Home Hospice Care, Veterans Outreach
Hospice of Santa Cruz County—$25,000
“We deeply appreciate Hospice Foundation’s support as we
provide excellent patient and family care in North Monterey
County, and with our outreach efforts with local hospice
providers in the We Honor Veterans program.”
—Michael Milward, CEO
Hazel Hawkins Palliative Care Team
Catholic Charities Grief Counseling
In-Hospital Palliative Care
Hazel Hawkins Memorial Hospital—$55,395
“For the last 10 years, Hospice Foundation has helped us
plan, launch and sustain our hospital’s palliative care services
for patients at the end of life. These professional services
combine pain and symptom control with emotional support,
and offer great peace of mind for families too.”
—Sunny Underwood, FNP, MSN, PhD, Palliative Care Coordinator
Emotional Support
Central Coast HIV/AIDS Services—$40,000
Education and Awareness about
End-of-Life Issues
Compassionate Care Alliance—$40,000
“We were all so inspired and energized by Dr. Byock, the
keynote speaker and presenter at our conference last year [pg. 5].
He presented concrete examples of how we all can work together
to help improve the delivery of end-of-life care locally, and
across our nation.”
—Vicki Nelson, Executive Director
Individual Grief Counseling
Catholic Charities—$20,000
“There are things in life we will never understand, and
HIV/AIDS will forever be one, but through this pandemic came
beauty-through the relationships between CCHAS and those
we serve. The long-time support of the Hospice Foundation is
permanently etched in our hearts-never to be forgotten. Thank
you for making the unbearable bearable and bringing light to
the darkest of times.”
—Katherine J. Thoeni, Executive Director
“Some people have a really difficult time in their grief and
need special interventions. With this funding, we are providing
counseling by a licensed mental health clinician to low-income
people who would otherwise not have access to any help. We
also provide training and supervision to social services graduate
students who are studying grief counseling.”
—Maria Runciman, Director of Mental Health Programs
Transportation, Family Support
In-Hospital Palliative Care
Children’s Hospice and Palliative Care Coalition—$50,000
Natividad Medical Center—$20,000
“With Hospice Foundation’s assistance, CHPCC’s program
helps transport seriously ill children and their families to and
from medical treatments at Lucile Packard Children’s Hospital
and UCSF Medical Center, safely and comfortably. This provides huge peace of mind for an increasing number of families
with limited resources.”
—Jennifer Ramirez, Deputy Director
“This grant is funding Natividad Medical Center’s palliative
care program and specialized training for staff in caring for
low-literacy patients and those who speak limited English.
The result will be the development of educational materials to
help each patient and family consider end-of-life issues and the
purpose and benefits of in-hospital palliative care services.

someone the artist wished to honor or remember, in keeping
with the theme of Hospice Foundation’s year-end campaign.
“We received a record number of entries this year,” said
Jennifer Pettley, Director of Communications for Hospice
Foundation. “All of them were fantastic-very heartfelt and
creative, so it made it especially hard for our judges to make
the selections.”
In addition to the Best of Show, three divisions of the

Improving Care at the End of Life
Grants provide a way for donors to fulfill their wish to support compassionate end-of-life-care services. Hospice Foundation awards these
grants only to the most efficient, well-managed non-profit organizations. Each grant recipient must submit a report semi-annually on
the use of their funding, and a final report that describes the progress made and outcomes from the grant funding they received.

The Importance of Estate Planning
“Estate planning?” you ask. “But isn’t that for the wealthy? I'm lucky to be keeping up with my mortgage payments and bills.”
Most people have this reaction when they hear the term
“estate planning.” But estate planning is not just for the
wealthy. It is for everyone. So maybe we should use the phrase:
“making your decisions about your assets,” whatever size or
type they may be. Your estate consists of everything you own,
including your home and its contents, business, financial
accounts, real estate, and other personal possessions.
Estate planning is the process of deciding what happens to
these items after your death, and describing those plans in the
appropriate legal documents, such as a Last Will and
Testament. And sooner is better than “some other time,”
because sometimes an accident, sudden illness, stroke or heart
attack can happen, causing unexpected death.
Most people have some assets and, even if they have only
one family member who should receive them, it really helps to
have that described in a will, just in case.
When there are no family members and you want the assets
to go to charity, the need to define this in a will is important
because state law won't know (or care), especially the charity
of your choice.
When there's more than one family member, a will can
clearly describe your wishes for distributing your assets, and
you can avoid leaving family disagreements behind you.
According to the National Association of Estate Planners
and Councils (www.naepc.org), more than 120 million
Americans do not have up-to-date estate plans to protect their
families. Many people procrastinate because they don't want to
think about their demise. Estate planning, however, can bring
them peace of mind.
Beyond a simple will, of course, there are myriad ways to
deal with one's assets. Through a well-crafted plan, you will
discover ways to reduce your taxes and still provide for those
people and causes you love. And, as your life circumstances
change, you may need to update your estate plans to make
sure they still describe your wishes for your family or others in
the foreseeable future.
Next time, when you hear the term estate planning, you’ll
be able to say, “In fact, we’ve just updated everything since the
recent change in our family situation. I feel so much better,
having taken care of it.”
Gain more peace of mind. Take action this week to make
plans for your estate. No time like the present.
