LIGHT DUTY (LD) PROCEDURES 1. 2. 3. 4. 5. 6. 7. 8. 9. 10. 11. 12. Works in conjunction with Workers’ Compensation HR receives M.D. statement with restrictions. Operators that have any type of restriction or brace/orthopedic device cannot drive. For operator, find out their driving schedule. A maximum of five (5) employees can be scheduled at one time. 8:00 a.m. – 4:00/(4:30 lunch) p.m. is the earliest shift. Can schedule in 4 or 8 hr. increments. One (1) LD assignment is allowed per injury. Maximum LD timeframe= 20 days or 160 hours. Call employee to notify of location/date/time to report to LD. Employee must wear uniform on LD assignment. E-mail Help-Transportation, Service Center, Vicki Learn/Chris Fanelli/ Sharon Remick], and Phalease Crichlow/Teresa Boone to notify of coach operator’s LD schedule. Include operator badge number on e-mail. E-mail Maintenance (or Operations, if applicable) copies of LD letter, Standard Operating Procedure, and general Workers’ comp letter. Medical restrictions from WC doctor are also included in the initial notification. Maintenance will make two copies of signed letter: 1 - HR; 1 - Employee. After signature is acquired, place copy in WC file. When LD ends, send tally sheet (light duty hours) to employee; keep copy for HR records. Take numbers from WC log. Project week out; can complete two (2) weeks prior to end of LD. When LD ends notify CorVel. Send CorVel salary information calculated by HR. When employee/coach operator is returning to regular duty, contact Help-Transportation, Service Center, Vicki Learn, Bob Williams, Chris Fanelli, and Sharon Remick; cc: Phalease/Teresa. HR calls Service Center for the employee’s work schedule. HR contacts employee, to notify of work schedule; employee is advised to contact Service Center to report in to work. Create WC file; complete Return to Work Checklist. Additional Notes: On Mondays, when employee is on LD, calculate time for Payroll. Maintenance or Operations will submit completed timecard. HR lists pay rate on spreadsheet and timecards every week. Copy timecards. AWW: Look back 52 weeks and divide by 52 (or actual number of weeks worked). AWW = AWW x 2 ÷ 3 ÷7 Light Duty = AWW + MWB ÷ 2 ÷ 40 (hrs./wk.) Requests for time off must be submitted to Phalease Crichlow/Lillian Motton, HR with “Employee Request for Time Off” form. For Health Maintenance requests, 48 hr. notice is required. Employees are given ½ hr. commute time before and after WC m.d. appt. One hour is allotted for m.d appt.; one half hour (each way) for commute. Therapy appts. must be scheduled at either the beginning or end of shift.
