PLEASE NOTE: The Educational Impact Statement must be completed and signed by a responsible person in the community (not
related to the applicant) who has been directly involved with the applicant and can provide information on the circumstances and the effect
on the applicant’s educational performance and reasons for discontinuing senior schooling.
