Helical SURECardio Prospective Aquilion PRIME
Background
A 68 year old female with known coronary artery disease and prior stent placement in the left anterior descending artery (LAD) is now
experiencing chest pain on exertion. Cardiac CT was requested to check stent patency.
Findings
The stent placed in the LAD is patent. There is no significant luminal stenosis in the left coronary system. There is an anomalous origin of
the right coronary artery (RCA) arising from the left coronary cusp. Multiple plaques are noted in the proximal and mid RCA causing mild
liminal stenosis. Effective dose for the scan was 0.8mSv.

Discussion
The wide spread adoption of CT to evaluate the coronary arteries has been accompanied by manufacturers developing techniques that
minimise radiation exposure. Prospective scan techniques that expose only for a small portion of the cardiac cycle are now favoured
over retrospective techniques for this reason. Some manufacturers have developed axial step and shoot techniques thereby abandoning
the well-known benefits of ECG gated helical scanning.
Toshiba’s unique solution was to develop SURECardio Prospective, the only prospective scan mode that enables the shorter scan times
and superior z-axis uniformity of a helical scan technique.
Cardio Prospective incorporates sophisticated arrhythmia detection software that continually monitors a patient’s heart rate during
the scan planning and the scan itself, identifies any irregularity in the patient’s rhythm and can change the scan mode on the fly to
adapt to this irregularity thereby ensuring robust results are obtained.

An ECG gated prospective helical scan was performed of the heart following an injection of 60mL of 350mg/mL concentrated iodinated
contrast media followed by a 50mL saline flush.
