ELBOW
Proceed to elbow using pads of two fingers on both sides of elbow
using circular motion massage soft spots for 90 seconds. When
stimulation is completed, manipulate joint with slow gentle stretch
until resistance is met. Apply orthotic while joint is in relaxed
position for long term effect.

WRIST
Proceed to wrist. Using pads of thumb and index fingers, massage soft spots on back and front of wrist using circular motion for
90 seconds. At the completion of the 90 second stimulation,
slowly and gently stretch wrist to extension. When resistance is
met, stop

HAND
Proceed to fingers. Work index fingers into each side of hand
massaging both inside and outer fingers for 90 seconds. When
both fingers meet, slowly and gently stretch fingers to extension.
When you meet resistance, stop. Hold in relaxed position and
apply W.H.O.

LOWER EXTREMITY
Working proximal to distal, using pads of middle and index finger,
massage the soft spot on the hip using a circular motion for 90
seconds. At the completion of the stimulation, slowly and gently
manipulate the hip joint until you feel resistance.
Leave hip joint in relaxed position.

KNEE
Proceed to knee using pads of two fingers on the soft spots on
both sides of the knee using circular motion for 90 seconds; at the
completion of the 90 second massage, manipulate the knee joint
by placing the resident’s heel in your hand and raise off the bed.
Using gravity and gentle pressure extend leg until you meet resistance. With the joint in a relaxed position apply orthotic for long
term effect.

FOOT
Proceed to foot. Using pads of two fingers find soft spots at the
back of the ankle and using a circular motion massage the points
for 90 seconds. At the completion of the 90 second stimulation,
manipulate the joint using slow gentle stretch until resistance is
met. With joint in relaxed position apply orthotic for long term
effect.

Massage Points
The massage takes 90 seconds of manual stimulation of the
neuromuscular junction of the joint. The joint should be
maintained in the relaxed or contracted position until the
90 seconds of massage stimulation have been completed.
After the 90 seconds of stimulation, slowly and gently
stretch the joint to the point of significant resistance only.
This position is the sub-maximal stretch desired. Hold the
joint in the extended position for a minute or two until you
feel the resistance to stretch lessen and stop. At this point,
apply orthotic and proceed to next affected joint.
The massage point is known as the “Soft Spot” at the neuromuscular junction. The “Soft Spot” ranges in size from a
quarter (hip) to that of a dime (wrist). Most soft spots provide a soft space with more resistive or bone surrounding
it. The soft spot is usually found between the bone and
tendon at the joint junction.
To stimulate the soft point, use a circular motion with the
pads of two fingers (index and middle) at the center of the
soft spot using gentle pressure on both sides of the joint
simultaneously. The stimulation must continue for the full
90 seconds to achieve the desired benefits. It may take
longer if severe tone and/or guarding exists. Always work
proximal to distal from point to point until the joint is manipulated to a sub-maximal stretch.
NECK & HEAD
Using the pads of two fingers (index & middle) perform a circular motion at the center of the soft spot using gentle pressure
for 90 seconds.
May take longer if severe tone exists. Maintain joint in a relaxed position until the end of the 90 second massage. Slowly
and gently bring the head to point of resistance (sub-maximal
stretch)

Massaging the soft tissue of the joint initiates blood flow to
the joint, releases tension in the joint, and stimulates the
production of synovial fluid, the body’s material lubricant,
all of which facilitates joint movement.
MSGS is most effective with a relaxed and participating
patient. Always discuss the techniques with the patient,
explaining what you are going to do and why and tell them
that you need their participation to achieve your goal and
with their participation it should be a pleasant experience
without pain or discomfort.
