Customer (person submitting a sample) registers at the guard house. 2. Customer proceeds to RSTL office at the second floor of the building and upon instruction of the guard on duty. 3. Sample Receiving Officer (SRO) checks if the sample to be submitted is valid and if a sterile sampling bottle is required. If the sample is valid: 1. If sample is valid and sterile bottle is not required, customer gives information to the SRO for the Test Request Form (TRF). 2. Upon completion of TRF, customer pays total cost of fees to the Cashier in the first floor and claims Official Receipt (OR) of payment. 3. Customer comes back to the RSTL office, shows the OR and gets duplicate copy of the TRF. 4. Customer claims Report of Analysis on the date due stated in the TRF. Reports of analysis are issued only to the person who submitted the sample or to an authorized representative. If a sterile bottle is required: 1. Customer gives information for the TRF upon instruction of SRO. 2. Customer pays to the Cashier at the 1st floor the partial payment of P600 for drinking water samples or P500 for environmental samples and gets OR of payment. 3. Customer goes back to the RSTL office, shows OR and claims sterile sampling bottle and gets instructions on sampling schedule, sampling procedure, and handling of samples. 4. Customer comes back to the RSTL to submit valid samples. If sample is invalid: Customer gets instructions on submission of valid samples and comes back on the scheduled date. Note: The customer replaces loss, breakage, or damage to borrowed sampling bottles. Customer (person submitting an item) registers at the guard house. 2. Customer proceeds to RML Office upon instruction of the guard on duty. 3. Receiving officer checks if the request is an on-site or in-house calibration. If On-Site: 1. Customer writes letter requesting for On-Site calibration schedule. Request should be addressed to the Regional Director. This request must state number of items, type, capacity, and other pertinent data. 2. The requesting party must bear the following: a) Transportation expenses (roundtrip) of the calibration officer. b) Porterage of equipment to be brought to and from the calibration site. c) Food and accommodation of the calibration officer. d) Other incidental expenses incurred during the conduct of the On-Site calibration. 3. Customer returns to RML to accompany staff to On-Site location on their scheduled date. 4. If an item(s) from RML needs to be borrowed, customer fills up item borrowers slip. 5. The customer then proceeds to the Work Request Form procedure*. 6. The requesting party should provide assistance, necessary tools and equipment during the calibration. 7. RML staff shall render services within regular office hours (8:00 AM – 5:00 PM, Monday to Friday). Overtime fee shall be charged if work is rendered beyond time specified. 8. In case calibration service could not be conducted or completed due to problems attributed to the customer, the customer must bear costs mentioned above if already incurred. 9. If customer has to fetch RML staff from the office, actual departure should be not later than 15 minutes after the agreed time, otherwise job will be rescheduled. If In-House: 1. The receiving officer checks if the item is valid for calibration. 2. Sample/equipment/instrument submitted for calibration should be clean and in good working condition. Sample not conforming to these requirements will not be received/calibrated. 3. If the item is not valid, the customer requests instructions for item validity. 4. Calibration services do not include repair. If sample/equipment/instrument cannot be calibrated due to defect, calibration shall be terminated. Customer shall be notified. 5. If the item is valid, the customer proceeds to the Work Request Form procedure*. 6. Instrument/equipment submitted for calibration should have accompanying equipment manual. Tank truck should have photocopies of Certificate of Registration (CR), previous calibration certificate, and engine and chassis stencil. *Work Request Form Procedure: 1. Customer and Item Receiving Officer fill up the Work Request Form (WRF). 2. Customer pays total cost of fees to the Cashier, and claims the Official Receipt (OR). 3. Customer comes back to Receiving Officer, and shows OR. 4. Customer returns borrowed items (if applicable) after calibration has been conducted. 5. Customer claims Certificate of Calibration and items on due date (An authorized representative may claim the certificate of calibration).
