This document has been created to help you plan your online entry.
All entries must be submitted through the online process

Your details:
You will be asked to provide details including Name, Address,
Contact number, Email, Date of Birth and your Unique Learner
Number (available from your learning provider or employer)
Apprenticeship details:
This section will ask you for information such as when you started
your apprenticeship. Also details of any qualifications you achieved
before starting this Apprenticeship (e.g. GCSEs). Other questions
include:
When did you complete or when do you expect to complete your
Apprenticeship?
Please list details of your achievements as a result of your
Apprenticeship to date, e.g. qualifications such as NVQs, technical
certificates, Key Skills or units towards qualifications.
Has your Apprenticeship resulted in you going on to further training?
If yes, please provide details.
Which type of Apprenticeship you are doing?
Your employer details:
This section asks for contact and address details of the organisation
you are employed by, including the name of your manager/supervisor
Your Learning Provider Details
You will need to complete this section if your training is provided by a
learning provider different
to your employer. It asks for a contact name, address, telephone and
email address
The following are the questions you will be asked on your
application form.
Question 1
Explain your role, key duties and responsibilities within the
workplace. (Max 200 words)
Entrants are asked to describe how their role fits within the
organisation they work. Judges are
looking to understand an outline of your job so they can understand
what your outstanding qualities are overall. Successful entrants
usually consider the following points in their answers:
– Provide an outline of your role (including key duties and
responsibilities) and how it fits within
the organisation.
– Clarify why you chose the Apprenticeship career route and how you
applied for it.
– Describe the team you operate within and your personal
contribution to team objectives and
team performance.
– A summary of your personal aspirations and planned career path.
Question 2
Please detail your contribution to the business objectives of
your employer, the impact this
has had on the organisation and the benefits to your employer.
(Max 300 words
Judges are looking for exceptional performance measures and to
establish how you have exceeded
expectations by going above and beyond your everyday duties. Try
to provide as much evidence
as possible (such as feedback; costs savings; initiatives;
innovations), in order to substantiate
your answers. Successful entrants usually consider the following
points in their answers:
– Explain how you have applied your learning in the workplace, and
provide examples of your
personal contribution that you consider demonstrates outstanding
performance.
– Describe how you have contributed to your employer’s overall
organisational success, using
specific examples and/or measures of your personal impact.
– Examples and supporting information which showcase your
performance achievements.
Question 3
Please detail the ways in which you have benefited from your
Apprenticeship, include
personal progression, any positive actions that have been taken
and anything that you have
personally achieved to get there. (Max 300 words)
Judges want to understand how your Apprenticeship has impacted
on your career and how it has contributed towards achieving your
ambition and goals. Give examples of how you have personally
benefited from your Apprenticeship. Successful entrants usually
consider the following
points in their answers:
– An outline of the skills you have developed since you began your
Apprenticeship, and how you
have applied these within (and perhaps outside) your role in the
organisation.
– Details of your greatest personal achievements to date, and why
you believe these are noteworthy.
– How your Apprenticeship has impacted on you, especially in terms
of your ambitions and
motivation.
Question 4
Why do you deserve to be named Apprentice of the Year? (Max
200 words)
The judges are looking for inspirational individuals, so consider this
question as your opportunity to
grab their attention and stand out from the others. Successful
entrants usually consider the following points in their answers:
– Why you believe that others can learn from your Apprenticeship
experiences, and tangible
examples of you currently providing a role model to others.
– Details of your activities as an ambassador for Apprenticeships and
vocational education both
within, and outside, your organisation.
– An overview of how, and what, you would do to promote
Apprenticeships if you were successful
in becoming the ‘Apprentice of the Year’ 2014.
Referee’s Question
Your referee should be your employer who will be asked to provide a
response to the following question:
Why should this person be nominated for the Apprentice of the
Year Award?(Max 250 words)
The referee should consider how the apprentice has demonstrated
commitment to their own
personal development and progression. The benefits they bring to
the organisation and any
inspirational qualities or outstanding contributions, and give clear
examples of achievement and
attainment in learning and work.
