regulatory-driven form changes, form enhancements, and new forms
that are being developed. You will receive a description of these
changes so that you can plan ahead for implementing them. We believe
this service enhances the value of our electronic forms offering while
providing the extra level of service you have come to expect from

U-1 - Statement of Purpose-Margin Stock-Bank
These forms are being revised. In accordance with applicable law, a lender is required to have a
Statement of Purpose for an Extension of Credit Secured by Margin Stock form completed and
signed when a lender extends credit in excess of $100,000 secured directly or indirectly, in whole
or in part, by any margin stock.
There are two versions of the form, one for banks and one for non-banks (U-1 and G-3). The forms
carry an expiration date in the upper right hand corner. The federal authorities have updated the
expiration date and the newest expiration date is now January 31, 2018. The forms have not been
changed substantively. The changes are an update of the approval number in the U-1 and an
extension of the approval date for both forms.

LPCA-2 - Participation Certificate and Agreement-2
The form LPCA-2, Participation Certificate and Agreement-2, is being revised. We are adding an
optional paragraph that provides that notwithstanding any other provision of the Agreement, in the
event of a Borrower default, the Seller and Purchaser will share Collections pro ratably. There will
be a checkbox for the paragraph.

The form 8300, Cash Report Over $10,000 (Trade or Business), is being revised to reflect
IRS/FinCEN changes to the form as of August, 2014. Changes are limited to instructions and
background information and do not substantially affect completion fields.
New - Mandatory Market
RS-GS-APP - Retail Sales Goods and Services Application
The form RS-GS-APP, Retail Sales Goods and Services Application, is being created. The application
is designed for consumer credit applications involving the credit sales or lease of goods and/or
services. "Goods and Services" is a generic reference to personal property and services and is not
designed for credit transactions secured by real property or dwellings. Dealers should review the
application to determine if this application is suitable for the types of goods and services dealer
provides. Certain products may require special state or federal disclosures, such as
telecommunications products or services. Dealers MUST review and determine if the credit
application sataisfies any compliance requirements that are unique to the dealer's business.

The form POA-F-PA, Power of Attorney-Financial, is being revised to more closely follow Section
5601 Title 20 (Decedents, Estates and Fiduciaries) of the Pennsylvania Consolidated Statutes. The
word "NOTICE" will now appear under the Power of Attorney title on page one. The agent
acknoledgement language has been revised and now is verbatim from the statute and lastly, the
witness signature lines have been moved directly under the principal signature line.

Vehicle Warranty Law is intended to provide certain warranty rights to consumers who purchase a
vehicle from a dealer. The BUYERSGUI-MA is similar to the Buyer’s Guide required by the Federal
Trade Commission (16 CFR 455.2) with the exception of state-specific language.

The form BOND-CASH-IN, U.S. Savings Bond Redemption, is being revised so that the Backup
Withholding Certifications conform with the most recent IRS substitute Form W-9 Instructions.
Specifically, we are adding the FATCA code certification language (which is technically required
even though a FATCA code would never apply to an account maintained in the U.S., at a U.S.
financial institution). In addition, the Wolters Kluwer Financial Services Style Guide is being
implemented.

ACHORIGAGRLZ - ACH Origination Agreement
The form ACHORIGAGRLZ, ACH Origination Agreement, is being revised: (a) to reflect a change to
the baseline threshold for unauthorized returns from 1% to 0.5% in keeping with an underlying ACH
System Rules change effective September 18, 2015; (b) to add flexibility to the timing of delivery
areas of the form; (c) to make the Authorized Signatures section optional; and (d) to add terms to
the Security Procedures area for greater consistency with similar provisions on other Wolters
Kluwer Financial Services documents subject to Uniform Commercial Code Article 4A. This form is
used as a service agreement when a business/commercial account holder arranges directly with the
financial institution for the ongoing origination of Automated Clearinghouse (ACH) transfers of
various types. These changes are recommended, not technically mandatory, as the contract has
always deferred to ACH Rules and has always anticipated ongoing changes to details like return
thresholds, and users have had other means, such as use of the Additional Terms area, to address
the odd case-specific scenario not accommodated by the standard options.

Updates to Signature Cards In 2015
Wolters Kluwer Financial Services has been in the process of revising account agreement forms that
contain the substitute Form W-9 certifications (the “Backup Withholding Certifications” section).
As part of these revisions, some of our older signature card products are being discontinued.
Changes to the Substitute Form W-9 Requirements
The revisions are necessary to comply with the IRS' December 2014 revisions to the "Instructions for
the Requester of Forms W-9,” a publication that includes the IRS' requirements for users of

substitute forms W-9. Communications during 2014 suggested that the most conservative means of
Form W-9 compliance since August 2013 has been to use the official, separate Form W-9 (Wolters
Kluwer Financial Services Form VMP9030) instead of relying on substitute form certifications. That
is still our recommendation until the revised signature cards are released in 2015.
Signature cards are being revised to conform to the new substitute W-9 Instructions, including
some new products and some discontinuations. In the revised backup withholding sections, we are
adding a technically-required FATCA code certification and a reference to exempt payee code, as
applicable. We are revising the business ownership type descriptions to capture the specific tax
classification detail as collected by the official Form W-9. We are also adopting the “singlesignature” design in most cases (using a single signature for both the account agreement and the
backup withholding certification rather than requiring an additional, separate signature just for the
certifications).
Note: While the IRS official form W-9 was itself revised in December 2014, the revisions to
the official form do not alter any substance of the certifications collected by the form, so
use of either the “December 2014” revision or the prior “August 2013” revision of the
official form should suffice for compliance during the interim.
Account Agreement Revisions
In order to accommodate these substitute W-9 changes, we are removing some account holder
identifying information previously collected on some forms, which is typically redundant with other
account documentation anyway. In addition, some Wolters Kluwer Financial Services’ Style Guide
enhancements are being implemented to some forms so that all designs accommodate standard bar
code and e-signature fields, as applicable.
The following table details the Preprinted and E-Forms signature card products that being revised
or added. Other signature card products are being discontinued.

JAR-LAZ-CT - Joint Account Report
The form JAR-LAZ-CT, Report as to Joint Account for Connecticut, is being discontinued without
replacement. The statute requiring this report to the CT Department of Revenue was repealed in
2001, and the state agency responsible for the content of the form has not maintained this or any

Discontinued (xx/xx/xx)
The document is being discontinued and replaced by another document as specified. The
discontinued document is still compliant; however, future revisions driven by legislative and
regulation imperatives will be rendered on the new version of the document, not the old version.
The implementation time frame by alliance and customer is discretionary, but it is incumbent on
the parties to implement in a timely manner so as to not jeopardize continued compliance.
OR
The form is being discontinued and no longer supported. There is no replacement form designated
and form will not be supported at specified date. The action could be driven by several causes such
as removal of compliance requirement or business decision to not support a line of business.
Enhancement
A change needed to enhance an existing form or to create a new form.

The new or revised document is being driven by legislative or regulatory mandate and is required
