Welcome to the SUBWAY School Lunch Order Program
To raise money for this year’s Grade 6 Graduation gift to the school, Grade 6 students are organising four
whole school Subway Lunch Days. The term 3 lunch will be on:
Tuesday 25th of August 2015
If your child would like to order a Subway Lunch, the attached Lunch Order form will need to be filled out
(one per child) and returned to your child’s class teacher with the correct money enclosed.
All orders are to be returned by Wednesday 19th of August 2015
Late orders will not be accepted.
The SUBWAY School Lunch has several choices. Please remember to write your child’s name and grade as
well as tick the relevant boxes for your child’s selection.
 Choose ONE bread type for your roll.
 Tick ONE meat selection.
 Tick as many salads as you want.
 Choose ONE sauce
For $6.00 your child will receive a 6 inch sub sandwich, a drink (water, apple juice or chocolate milk) and 1
side (apple pieces, yoghurt or chocolate chip cookie).
Please ensure you enclose the correct amount of money as no change is given.
