An Incident Report Form
should be completed for:
Any staff-related accident
or situations i.e. a staff
member experiences verbal or physical aggression
or any event which may
have caused injury to the
employee.

deviation of policy, fire,
theft, falls that are experienced by patients and visitors, medication incidents
including close calls, etc.
Immediately after the staff

review who will then forward it to the Director
Quality/Risk for Risk Reporting.

or concerns regarding the
above information please

contains the scores and comments
made by both focus groups and by
individual participants. The overall
report was extremely positive. There
are many manifestations of deep
commitment to patient care in all its
aspects, with a special recognition of
A few weeks prior to the site visit by
the CHAC External Consultant, Mr.
Robert Stewart, participants were
given a survey questionnaire to com-

and staff, and a deep yearning to be

organization as well as from the communities it serves.
